Position: - Care Management Support Coordinator II (235521)
Location Address: Remote CST
Anticipated Start Date: 3/24/2025
Duration: 06+ Months (with possible extension)
Shift: Monday - Friday 8:00 AM to 5:00 pm CST
Pay Rate: $23.00/hr on W2
Position Purpose:
The Care Management Support Specialist plays a key role in supporting administrative care management activities, including member outreach, inbound call handling, and service scheduling. This position serves as a primary point of contact for members, providers, and internal teams to address inquiries, resolve issues, and maintain accurate member records in compliance with state and regulatory guidelines. Key Responsibilities:
- Conduct outreach to members via phone to assist with care plan next steps, community resources, and health plan services.
- Support members by connecting them to appropriate health plan and community resources, ensuring high-quality customer service.
- Apply working knowledge of assigned health plan activities and available resources.
- Serve as the first point of contact for member and provider inquiries, explaining care plan procedures and protocols.
- Assist with member onboarding, including sending welcome letters, correspondence, and educational materials to support positive member-provider relationships.
- Maintain accurate and up-to-date non-clinical member records in accordance with state and regulatory requirements.
- Identify available benefits and local resources, making referrals to address Social Determinants of Health (SDOH) needs.
- Perform additional duties as assigned while adhering to company policies and standards.
- High School Diploma or GED required.
- 1 - 2 years of related experience in healthcare administration, customer service, or a similar role.
- Strong communication and problem-solving skills with the ability to handle member and provider inquiries professionally.
- Basic knowledge of healthcare benefits, care plans, and community resources.
- Proficiency in administrative tasks, record-keeping, and documentation.
- Ability to comply with all policies, procedures, and regulatory requirements.
