BCforward is currently seeking a highly motivated Community Resource Coordinator II for an opportunity in Atlanta GA 30339!
Position Title: Community Resource Coordinator II
Location: Atlanta GA 30339

Expected Duration: 3 months of contract

Pay Range: [$20.00] - [$25.00]Please note that actual compensation may vary within this range due to factors such as location, experience, and job responsibilities, and does not encompass additional non-standard compensation (e.g., benefits, paid time off, per diem, etc.).
Requirements:- Experience in community outreach, social work, or related fields
- Bilingual proficiency
- CERTIFICATION as a Community Health Worker (CHW)
- Member Support:
- Connect members to known community and care resources in a cost-effective manner.
- Conduct non-medical assessments (e.g., home safety, transportation, and employment) to identify and address care barriers.
- Assist members in navigating community resources and provide health education when appropriate.
- Conduct outreach via phone and in-person visits, including locating hard-to-reach individuals.
- Community Engagement:
- Build partnerships with community organizations to address member needs.
- Coordinate and participate in community events across designated regions, potentially extending statewide.
- Documentation and Compliance:
- Maintain thorough records of community resources in alignment with health plan requirements.
- Document member interactions and assessments to support clinical team efforts.
- Team Collaboration:
- Work closely with clinical teams, including nurses and social workers, to provide non-clinical support.
- Support member-related correspondence and educational material development.
- High School Diploma or equivalent.
- 1-2 years of relevant experience in community outreach, social work, or related fields.
- Reliable transportation and willingness to travel extensively (80% field-based).
- Bilingual proficiency.
- Certification as a Community Health Worker (CHW).
- Knowledge of community resources within Georgia.
- Experience in managed care or healthcare environments.
- Strong communication, organizational, and service coordination skills.
- Training: Mandatory compliance and role-specific training.
- Metrics: Success will be measured by a target of 60% successful home visits per month.
- Internal and External Interactions: Frequent collaboration with members, community organizations, and internal clinical teams.
