BC Forward is looking for Care Management Support Coordinator II (Remote) PST

Position Title: Care Management Support Coordinator II (Remote)

Locations: REMOTE - PST (NO California candidates are applicable)
Anticipated Start Date: 04/07
Expected Duration: 5 Months
Job Type: Contract with possible extension
Shift: 8 AM - 5 PM PST (No Overtime)
Requirement: Must be able to work PST hours. California candidates are not eligible due to additional requirements related to pay, schedule, and compliance.

Pay Rate $22.00/hr. on W2
Need: Minimum of 18 months of experience in a health plan or healthcare-related industry AND Outbound Calling.


Job Description:
We are part of Care Management, Shared Services, focusing on Medicare members. Our team plays a crucial role in improving member health by conducting outreach calls to close care gaps and ensure timely preventative visits. Additionally, we assist members in completing their Health Risk Assessments (HRAs) when due.
Purpose of the Team:
  • Conduct daily outbound calls to Medicare members to address care gaps.
  • Assist members with scheduling preventative visits and completing HRAs.
  • Collaborate with Primary Care Providers (PCPs) to obtain valid contact information when necessary.
Typical Day in the Role
Schedule & Expectations:
  • Full-time position with structured daily call assignments.
  • Potential for overtime based on business needs.
Key Responsibilities:
  • Outreach to at least 30 members daily, totaling 125 per week and 600 per month.
  • One call attempt per day to the listed number; if invalid, attempt contact through the PCP.
  • Discuss care gaps, review HRAs, and offer scheduling assistance.
  • If the member is unreachable, make four additional attempts at different times of the day.
  • Maintain an audit score of 90% or higher.
  • Meet the 10-day turnaround time (TAT) requirement.
  • Accurately document call interactions.
  • Make 80-100 calls per day.

Candidate Requirements
Education & Certification:
  • Required: High School Diploma or GED (Must be verifiable).
  • Preferred: N/A.
Experience & Qualifications:
  • Required: Minimum of 18 months of experience in a health plan or healthcare-related industry.
  • Disqualifiers: Candidates from California will not be considered per management request.
Performance Expectations:
  • Daily outreach to 30 new members.
  • Maintain an audit score of 90% or higher.
  • Meet the 10-day TAT for follow-ups.
  • Accurately document call interactions.
  • Complete 80-100 calls per day.
