BCforward is currently seeking a highly motivated Care Coordinator II for an opportunity in REMOTE.
Job Details:
Job Title: Care Coordinator II (REMOTE)
Location: Remote
Duration: 3 months of contract
Shift: 8am-5pm CST with an hour lunch

Job Description:
Supports care management activities and assists teams assigned to members to ensure healthcare providers and partners deliver services effectively, promoting continuity of care and member satisfaction. Engages with members through telephonic outreach or home visits, documenting care and service plans.- Conducts outreach via phone or home visits to engage members, discuss care and service plans, outline next steps, provide resources, address concerns, and offer ongoing education as appropriate.
- Coordinates care activities based on the care/service plan, collaborating with healthcare providers, community partners, and caregivers to adjust services as needed.
- Serves as a point of contact for member and provider inquiries, requests, or concerns related to the care/service plan.
- Communicates with care managers, practitioners, and other stakeholders to facilitate member services and ensure seamless care coordination.
- May assist in conducting service assessments and screenings, documenting member care needs.
- Ensures accurate documentation and maintenance of member records in compliance with state and regulatory requirements, distributing information to providers as needed.
- Adheres to standards of practice, contractual requirements, and regulatory guidelines.
- Identifies member needs and makes referrals to Care Managers, community-based organizations, or Disease Managers as appropriate.
- Educates members on benefits and available resources.
- Performs other duties as assigned.
- Complies with all policies and standards.
