BCforward is currently seeking a highly motivated Care Management Support Coordinator II for an opportunity in Remote-AZ!

Pay Range: [$22] - [$24.75]

  • Knowledge of medical terminology, Medicare/Medicaid services, and referral processes.

Job Description:

Position Purpose:

The Care Management Support Coordinator II supports administrative care management activities by performing outreach, answering inbound calls, and scheduling services. This role serves as a primary point of contact for members, providers, and staff to address inquiries and concerns while ensuring compliance with state and regulatory guidelines.

Key Responsibilities:

  • Conduct outreach calls to members to provide assistance with care plans, resources, and scheduling.
  • Support members by connecting them with health plan and community resources.
  • Serve as a front-line support for member and provider inquiries, explaining care plan procedures and protocols.
  • Facilitate member onboarding by distributing welcome letters, correspondence, and educational materials.
  • Document and maintain non-clinical member records in compliance with policies and regulations.
  • Assist in referrals related to Social Determinants of Health (SDOH), including food assistance, financial support, and medical services.
  • Ensure adherence to company policies and standards.
  • Perform other duties as assigned.
Minimum Qualifications:

  • Education: High school diploma or GED required.
  • Experience: 1-2 years of related experience in customer service, healthcare, or social services.
  • Technical Skills: Proficiency in Citrix, Microsoft Teams, and Excel. Typing speed requirement applies.
Preferred Experience:

  • Prior experience in a managed care setting, medical office, or facility.
  • Knowledge of medical terminology, Medicare/Medicaid services, and referral processes.
  • Strong multitasking skills and ability to navigate multiple systems.
Additional Requirements:

  • Language: Bi-lingual (Spanish) candidates preferred.
