This role involves member outreach, inbound call handling, appointment scheduling, and community resource coordination. The coordinator serves as a primary point of contact for members and providers, ensuring care gaps are closed and healthcare services are easily accessible.Key Responsibilities
Experience: 2-3 years in a managed care setting, medical office, or healthcare environment.
Skills:
- Serve as the first point of contact for members and providers, addressing inquiries and concerns.
- Support member onboarding by sending welcome letters, educational materials, and correspondence.
- Maintain and update non-clinical member records, ensuring compliance with state and regulatory requirements.
- Assist in identifying Social Determinants of Health (SDOH) needs and make appropriate referrals.
- Conduct daily outreach calls to members, providers, and pharmacies.
- Assist members with appointment scheduling, PCP selection, and care coordination.
- Screen members for eligibility to enroll in Care Coordination services.
- Enter assessments and notes into the charting system accurately and timely.
- Work collaboratively with healthcare professionals, providers, and community resources to facilitate member care.
- Ensure compliance with company policies, state regulations, and industry standards.
- Perform additional administrative duties as assigned.
Experience: 2-3 years in a managed care setting, medical office, or healthcare environment.
Skills:
- Strong communication and problem-solving skills.
- Experience handling high call volumes (50+ calls per day).
- Proficiency in Microsoft Office (Excel, Word, Outlook).
- Knowledge of medical terminology and general healthcare processes.
- Preferred Experience: Familiarity with SAP, JD Edwards, or other healthcare systems.
