Care Management Support Coordinator II
BCforward is currently seeking a highly motivated Care Management Support Coordinator II Remote
Position Title: Care Management Support Coordinator II
Location: Remote
Anticipated Start Date: 2/10/2025

Expected Duration: 3+ months (with possibility of extension or conversion)
Pay Range: $23/hr
Job Summary:
The Administrative Care Management Support Specialist is responsible for supporting administrative activities related to care management, including outreach, answering inbound calls, scheduling services, and serving as a key point of contact for members, providers, and staff. This role is critical in ensuring that members receive high-quality customer service, facilitating effective communication, and maintaining accurate member records in compliance with state and regulatory guidelines.
Key Responsibilities:
- Perform outreach to members via phone to support care plan next steps, provide information on community or health plan resources, and address questions or concerns related to scheduling and ongoing education for members and providers.
- Provide assistance to members in accessing health plan and community resources, ensuring they receive appropriate services and care.
- Serve as the first point of contact for member and provider inquiries, explaining care plan procedures, protocols, and addressing concerns.
- Assist with member onboarding, including sending out welcome letters, educational materials, and other correspondence to foster a successful member-provider relationship.
- Document and maintain accurate, non-clinical member records in accordance with state regulations and health plan guidelines. Ensure records are shared with providers as necessary.
- Utilize knowledge of local benefits and resources to make referrals to address Social Determinants of Health (SDOH) and other needs.
- Ensure compliance with all policies, procedures, and regulatory standards.
- Perform other duties as assigned to support care management activities and organizational objectives.
Education/Experience Requirements:
- High School diploma or GED required.
- 1-2 years of related experience in a customer service or administrative role, preferably within healthcare or care management.
- Familiarity with health plan operations and available community resources is a plus.
Skills & Qualifications:
- Excellent communication skills, both verbal and written, with the ability to interact professionally with members, providers, and staff.
- Strong organizational skills and attention to detail.
- Ability to maintain confidentiality and handle sensitive information in accordance with regulatory guidelines.
- Basic knowledge of healthcare programs, benefits, and resources.
- Proficiency in Microsoft Office and ability to learn new software systems.
- Compassionate and empathetic approach to customer service and care management.
