Typical Day in the Role
Daily Schedule & Overtime Expectations:
- Standard work hours: 8 AM - 5 PM PST
- Overtime is not required or expected
- Assigned 30 new Medicare members daily for outreach
- Make one call attempt per day to the listed number for each member
- If the number is invalid, also call the member's Primary Care Provider (PCP) to obtain a valid number
- If the number is invalid, also call the member's Primary Care Provider (PCP) to obtain a valid number
- If the member is reached:
- Review Care Gaps
- Complete a Health Risk Assessment (HRA) if needed
- Assist with scheduling appointments
- Ensure all questions or concerns are addressed

- If the member is not reached, make up to four additional call attempts at different times of the day

Position Purpose:
The Administrative Care Coordinator supports the care management team by performing member outreach, managing inbound inquiries, and coordinating services. This role serves as a key point of contact for members, providers, and internal staff to ensure timely resolution of issues and accurate documentation of interactions in compliance with state and regulatory requirements.
Key Responsibilities:
- Conduct outreach to members by phone to assist with care plan steps, provide education, address concerns, and connect members with available health plan or community resources.
- Support scheduling of services and ongoing communication with both members and providers to ensure a smooth care experience.
- Serve as the first point of contact for member and provider inquiries, offering support and clarification regarding procedures, benefits, and protocols.
- Facilitate member onboarding, including sending welcome letters, educational materials, and other correspondence to promote understanding and engagement.
- Maintain accurate, non-clinical documentation in member records according to compliance standards and provide updates to providers as necessary.
- Apply working knowledge of health plan operations, benefits, and local resources to identify and refer members to services that address Social Determinants of Health (SDOH).
- Ensure all communication and actions comply with internal policies, state regulations, and industry best practices.
- Perform other related administrative duties as assigned.
Education & Experience:
Required:
- High School diploma or GED (must be verifiable)
- 1-2 years of related experience in a healthcare, insurance, or customer service setting
- Experience in outreach or care coordination within a managed care or health plan environment
- Familiarity with medical terminology and basic understanding of care management protocols
