BCforward is currently seeking a highly motivated Bilingual Care Coordinator II - REMOTE for an opportunity in Tampa, FL.
Job Details:Job Title: Care Coordinator II - REMOTE Location: Tampa, FLDuration: 3+ months of contract Pay Rate: $25/hr. on W2Anticipated Start: ASAP **** Candidate need located in Tampa, FL OR nearby ***** Job Description:Supports care management activities and the teams assigned to members to ensure services are delivered by healthcare providers and partners, achieving continuity of care and member satisfaction. Engages with members through telephonic outreach or home visits and documents the plan for care/services.- Provides outreach to members via phone or home visits to engage them in discussions about their care/service plan, including next steps, available resources, questions, concerns related to recommended care, and ongoing education as appropriate.
- Coordinates care activities based on the care/service plan, working with healthcare and community providers, partners, and members/caregivers to accommodate changes or progress as needed.
- Serves as support for various member and provider inquiries, requests, or concerns related to the care/service plan.
- Communicates with care managers, practitioners, and others as needed to facilitate member services and ensure continuity of care.
- May assist in performing service assessments/screenings for members and documenting their care needs.
- Supports documentation and maintenance of member records in accordance with state and regulatory requirements, ensuring distribution to providers as needed.
- Adheres to standards of practice and policies in compliance with contractual requirements and regulatory guidelines.
- Identifies needs and makes referrals to Care Managers, community-based organizations, and Disease Managers.
- Provides education on benefits and available resources.
- Performs other duties as assigned.
- Complies with all policies and standards.
- Requires a high school diploma or GED.
- 2+ years in healthcare / managed care background
