BCforward is currently seeking a highly motivated Care Coordinator II for an opportunity in Remote - FL!

Job Description:

Position Purpose

The Care Coordinator II supports care management activities and care teams assigned to members, ensuring the timely delivery of services by healthcare providers and partners. This position focuses on member outreach and engagement to drive continuity of care and overall member satisfaction. Outreach is conducted primarily via telephone, with potential for home visits. The Care Coordinator documents services and care plans and ensures compliance with regulatory and organizational guidelines.

Key Responsibilities:

  • Conduct member outreach via phone or home visits to engage in care plan discussions, education, and follow-up.
  • Coordinate activities per the care/service plan in collaboration with members, caregivers, healthcare providers, and community resources.
  • Respond to inquiries from members and providers regarding care/service plans.
  • Collaborate with care managers and practitioners to ensure continuity of care.
  • Assist with member assessments and screenings, documenting care needs.
  • Maintain accurate, up-to-date member records per regulatory requirements.
  • Identify and refer needs to Care Managers, Disease Managers, and community organizations.
  • Educate members on health benefits, services, and community resources.
  • Submit daily productivity reports through Outlook.
  • Perform other duties as assigned.
  • Adhere to all compliance policies and standards.

Candidate Requirements:

Education & Experience:

  • Required: High School Diploma or GED
  • Required: 2+ years in healthcare or managed care (non-negotiable)
  • Preferred: Previous work-from-home experience
  • Preferred: Bilingual in Spanish or Creole
Skills & Competencies:

  • High volume call experience (50-75 calls/day)
  • Comfortable with outbound/inbound calling
  • Strong research and follow-up skills
  • Ability to multitask while documenting member interactions
  • General technical proficiency (Excel, Outlook, Word, Microsoft Teams)
  • Independent, self-motivated, and professional in remote work environment

Work Environment & Expectations:

  • Remote role but must have a professional home office setup.
  • In-person meetings may be requested quarterly (Tampa Bay area preferred).
  • If unable to work remotely (e.g., due to power/internet outages), must report to the Tampa office.
  • Training includes 1:1 preceptorship and weekly supervisor check-ins during onboarding.
  • Strong team culture with open communication and ongoing support.

  • Consistent daily reporting and accountability
