Care Management Support Coordinator II
- Member Outreach:
- Conduct outreach calls to members to support care plan next steps, provide ongoing education, and connect them with community or health plan resources.
- Assist members with questions or concerns related to scheduling services, care plan procedures, and protocols.
- Provider and Member Support:
- Serve as a front-line support for various member and provider inquiries, including scheduling services, explaining care protocols, and resolving any issues related to services provided.
- Provide support for member onboarding and facilitate the relationship between members and providers by sending welcome letters and educational materials.
- Documentation & Record Maintenance:
- Accurately document and maintain non-clinical member records in compliance with state and regulatory guidelines.
- Provide documentation and support to providers as needed.
- Social Determinants of Health (SDOH):
- Identify and address SDOH needs by referring members to appropriate community or health plan resources to ensure high-quality care and services.
- Administrative Duties:
- Perform various administrative tasks such as managing call logs, entering assessments into the charting system, and scheduling appointments for members.
- Education:
- High School diploma or GED required.
- Experience:
- 1 - 2 years of related experience in a medical, managed care, or healthcare setting.
- Experience with high call volume (50+ inbound/outbound calls per day).
- Member or patient-facing experience is required.
- Skills:
- Strong communication skills, both written and verbal, to effectively engage with members and providers.
- Proficiency in computer literacy, including data entry, charting systems, and basic office software.
- Ability to manage multiple tasks and priorities effectively.
- Experience:
- 2-3 years in a managed care setting, medical office, or healthcare facility with administrative duties.
- Knowledge of medical terminology is a plus.
