BCforward is currently seeking a highly motivated Care Management Support Coordinator II at Remote.

Position Title: Care Management Support Coordinator II

Location: Remote DE

Anticipated Start Date: 31 March 2025

Expected Duration: 4+ Months

Daily Schedule: 8:00 AM to 5:00 PM

Job Type: [FULL TIME (>%3D40 HRS WEEKLY)/ [TEMP], [Remote]

Pay Range: $22/hr

Please note that actual compensation may vary within this range due to factors such as location, experience, and job responsibilities, and does not encompass additional non-standard compensation (e.g., benefits, paid time off, per diem, etc.).

Position Purpose:

The Care Management Support Coordinator is responsible for supporting administrative care management activities, which include performing outreach to members, answering inbound calls, scheduling services, and assisting with care plan next steps. This role serves as a primary point of contact for members, providers, and staff to resolve inquiries, document member records, and ensure compliance with state and regulatory guidelines. The coordinator will also provide education, connect members to health plan resources, and manage administrative tasks related to member care.

Key Responsibilities:

  • Member Outreach:
    • Conduct outreach calls to members to support care plan next steps, provide ongoing education, and connect them with community or health plan resources.
    • Assist members with questions or concerns related to scheduling services, care plan procedures, and protocols.
  • Provider and Member Support:
    • Serve as a front-line support for various member and provider inquiries, including scheduling services, explaining care protocols, and resolving any issues related to services provided.
    • Provide support for member onboarding and facilitate the relationship between members and providers by sending welcome letters and educational materials.
  • Documentation & Record Maintenance:
    • Accurately document and maintain non-clinical member records in compliance with state and regulatory guidelines.
    • Provide documentation and support to providers as needed.
  • Social Determinants of Health (SDOH):
    • Identify and address SDOH needs by referring members to appropriate community or health plan resources to ensure high-quality care and services.
  • Administrative Duties:
    • Perform various administrative tasks such as managing call logs, entering assessments into the charting system, and scheduling appointments for members.

Required Qualifications:

  • Education:
    • High School diploma or GED required.
  • Experience:
    • 1 - 2 years of related experience in a medical, managed care, or healthcare setting.
    • Experience with high call volume (50+ inbound/outbound calls per day).
    • Member or patient-facing experience is required.
  • Skills:
    • Strong communication skills, both written and verbal, to effectively engage with members and providers.
    • Proficiency in computer literacy, including data entry, charting systems, and basic office software.
    • Ability to manage multiple tasks and priorities effectively.

Preferred Qualifications:

  • Experience:
    • 2-3 years in a managed care setting, medical office, or healthcare facility with administrative duties.
    • Knowledge of medical terminology is a plus.

  • Patient Assessment: Ability to assess and identify member needs and connect them to appropriate resources.
  • Communication: Clear and effective communication with both members and providers.
  • Computer Literacy: Proficiency in using software applications for scheduling, documentation, and communication.
