BC Forward is Looking for Care Coordinator II Remote

Job Title: Care Coordinator II
Location: Remote - Kentucky (Eastern or Western KY required)
Start Date: September 8, 2025
Contract Length: 3 months (with potential to extend or convert to full-time)
Pay Rate: $23/hour
Work Schedule: Monday - Friday, 8:30 AM - 5:00 PM (EST or CST, based on KY location)
Training: 2 weeks, 8:30 AM - 5:00 PM EST (Cameras on, shadowing sessions included)



Position Summary:

The Care Coordinator II supports care management efforts for vulnerable Medicare members to ensure continuity of care, compliance with preventive screenings, and overall member satisfaction. The role includes conducting telephonic outreach to engage members, assess needs, close care gaps, and provide education on benefits and available resources.

This position is entirely remote within Kentucky and focused on telephonic outreach only (no fieldwork required).



Key Responsibilities:

  • Perform telephonic outreach to Medicare members to engage them in care coordination and close open care gaps (e.g., HEDIS measures).
  • Discuss care/service plans and assist with scheduling provider appointments.
  • Provide ongoing education and support regarding benefits, services, and available resources, including addressing Social Determinants of Health (SDoH).
  • Collaborate with healthcare providers, community partners, and internal care management teams to ensure coordinated care.
  • Document member interactions and updates to care/service plans in compliance with regulatory and organizational standards.
  • Respond to member and provider inquiries regarding care plans, next steps, and follow-up services.
  • May assist in completing service assessments or screenings based on member needs.
  • Identify member needs and escalate referrals to Care Managers or Disease Management as appropriate.
  • Maintain accurate and timely member records, adhering to state and regulatory requirements.
  • Follow all Centene policies, compliance standards, and care coordination protocols.
  • Participate in weekly team office hours, monthly 1:1 check-ins, and maintain active communication via group chat and resource folders.


Minimum Qualifications:

  • Education: High School Diploma or GED (required)
  • Experience: 1 - 2 years of relevant experience (e.g., call center, provider office)
  • Skills:
    • Proficiency in Microsoft Word and Excel
    • Strong multitasking ability (navigate multiple systems while engaging members)
    • Knowledge of medical terminology
    • Strong verbal communication and active listening skills
  • Location: Must reside in Kentucky (Eastern or Western KY preferred)


Preferred Qualifications:

  • Prior experience with HEDIS or similar quality/compliance programs
  • Experience in healthcare, social services, or managed care
  • Familiarity with telephonic outreach in a healthcare setting


Performance Expectations:

  • Minimum of 10 member outreach calls per hour
  • Consistent documentation of calls and assessments
  • Address and close open care gaps as part of the care plan
  • Maintain professionalism and member-centered communication during all interactions
