Shift (Start/End Time): Monday- Friday 8am-5pm PST or MST

Position Purpose:
Job Description: Administrative Care Management Support Coordinator

Position Summary: The Administrative Care Management Support Coordinator provides essential administrative support to care management activities, including performing outreach, answering inbound calls, and scheduling services. This role serves as a key point of contact for members, providers, and staff, resolving issues and ensuring the documentation of member records in accordance with state and regulatory guidelines.

Key Responsibilities:

  • Provide outreach to members via phone to support care plan next steps, community or health plan resources, and address any questions or concerns related to scheduling.
  • Offer ongoing education to both members and providers throughout the care/service process.
  • Connect members to health plan and community resources to ensure access to high-quality customer care and service.
  • Serve as the front-line support for various member and/or provider inquiries, requests, or concerns, explaining care plan procedures and protocols.
  • Support member onboarding and handle day-to-day administrative tasks, including sending out welcome letters, program educational materials, and related correspondence.
  • Document and maintain non-clinical member records in compliance with state and regulatory requirements, ensuring all information is accessible to providers as needed.
  • Apply knowledge of benefits, resources, and Social Determinants of Health (SDOH) to make appropriate referrals for members.
  • Perform additional duties as assigned while ensuring compliance with all policies and standards.
Education/Experience Requirements:

  • High School diploma or GED required.
  • 1-2 years of related experience in a healthcare or customer service role is preferred.
Knowledge and Skills:

  • Strong communication skills, both written and verbal.
  • Knowledge of local benefits and resources to address member needs.
  • Familiarity with regulatory and state requirements for documenting member records.
  • Ability to manage time effectively and handle multiple tasks simultaneously.
