BCForward is looking for Care Coordination Support Specialist- Remote- SC
Job Title: Care Coordination Support Specialist
Location: Remote (South Carolina)
Duration: 06+ Months
Potential to Extend- YES
Hours: 40 hours per week, Monday to Friday (8:00 AM - 5:00 PM)
Start Date: ASAP
Hourly Rate: $22.00 - $23.00 per hour
Position Purpose:
The Care Coordination Support Specialist works closely with the senior care management team to assist in care management activities, ensuring that healthcare services are provided to members efficiently and effectively. This role serves as a liaison between care managers, providers, and members, coordinating care, performing member outreach, and addressing concerns to ensure continuity of care and member satisfaction.
Key Responsibilities:

Member Outreach & Engagement:
Perform outreach to members via phone calls or home visits to discuss their care plans, address questions or concerns, and provide ongoing education throughout the care process. - Ensure that all members are engaged and informed about their care plans and the next steps.
- Assist in scheduling appointments and facilitating access to necessary resources.

Care Plan Coordination:
Coordinate with healthcare providers and community partners to accommodate changes or updates to care plans. - Ensure that care plans are adjusted as needed and communicated clearly with providers and members.

Documentation & Record Keeping:
Maintain accurate and up-to-date records of members' care needs and services, ensuring compliance with state and regulatory guidelines. - Document service assessments, screenings, and other necessary information.
- Ensure proper documentation is shared with providers when needed.

Issue Resolution:
Act as a support resource for members and providers, addressing any concerns or issues related to the care plan or service plan. - Escalate more complex cases to care management or appropriate leadership.

Member Education & Resource Referral:
Educate members on benefits and available resources to enhance their overall care experience. - Make referrals to care managers or community-based organizations when necessary.

Collaborative Communication:
Work closely with care managers, healthcare practitioners, and other team members to ensure smooth coordination of care and follow-up on member progress. - Regularly update the team on the status of members' care and ensure no gaps in service.

Compliance & Safety:
Follow all organizational policies, standards, and regulations, ensuring compliance with contractual requirements and regulatory guidelines. - Promote safe practices and lead by example in fostering a safe work environment.

Other Duties as Assigned:
Support team initiatives, assist with training and development activities, and contribute to the overall success of the care management team. - Perform any additional tasks or responsibilities as needed to support the team and members.
Required Qualifications:
- Education:
- High School Diploma or GED required.
- Bachelor's degree in Nursing, Social Sciences, Social Work, or related field preferred for specific contracts (e.g., Illinois Youth Care Contract).
- Experience:
- 2-4 years of related experience in healthcare or care coordination.
- Experience in customer service or member communications, preferably in healthcare settings.
- Experience in Medicaid, Medicare, or HEDIS standards a plus.
- Skills & Competencies:
- Strong verbal and written communication skills.
- Proficiency with Microsoft Office Suite (Excel, Word, PowerPoint).
- Ability to work independently and as part of a collaborative team.
- Strong organizational and time management skills.
- Ability to handle sensitive and confidential information.
Preferred Qualifications:
- Experience:
- Previous experience in a customer service or healthcare setting working with members and providers.
- Familiarity with HEDIS, STARS, CAHPS, or NCQA guidelines is highly desirable.
- Certifications:
Any relevant healthcare certifications (e.g., Care Coordination, Healthcare Management)
