Care Management Support Coordinator II
BCForward is looking for Care Management Support Specialist- Remote
Job Description: Care Management Support Specialist
Location: Remote
Duration: 06+ Months
Possibility to Extend/Convert: Yes
Pay Rate: $25.00/hr on W2
Shift Timings: 8:00 AM-5:00 PM CST / 9:00 AM-6:00 PM EST
Position Purpose:
Supports administrative care management activities including performing outreach, answering inbound calls, and scheduling services. Serves as a point of contact to members, providers, and staff to resolve issues and documents member records in accordance with current state and regulatory guidelines.
Primary Responsibilities:
- Provide outreach to members via phone to support care plan next steps, resources, and scheduling
- Assist members with accessing health plan and community resources
- Serve as front-line support for member/provider inquiries, explaining care plan procedures
- Support member onboarding by sending welcome letters, correspondence, and program materials
- Document and maintain non-clinical member records per regulatory requirements
- Refer members to local resources to address Social Determinants of Health (SDOH)
- Perform other duties as assigned
Education/Experience:
- Requires a High School diploma or GED
- Requires 1-2 years of related experience
- Preferred: BA degree (not required)
Requirements:
- 2 years customer service experience
- Moderate computer skills
- Beginner Excel skills
- Experience in managed care or medical office/facility settings
- Ability to navigate computer systems and use Microsoft Office
- Strong communication, time management, and organizational skills
