Lead Patient Care Coordinator BC Forward is Looking for Lead Patient Care Coordinator in Nationally Remote Job Title: Lead Patient Care Coordinator
Location: Nationally Remote (Worksite: Houston, TX 77043)
Pay Rate: $19.04/hr
Assignment Duration: 5 Months
Start Date: April 8, 2025
Work Schedule: Monday-Friday | 10:00 AM-6:00 PM CST (1-hour lunch) or 10:30 AM-6:00 PM CST (30-minute lunch)
Possibility to Extend or Convert: Yes Position PurposeThe Lead Patient Care Coordinator will support the Supervisor in overseeing daily operations, mentoring team members, handling escalations from patients and providers, and ensuring efficient patient care coordination. This position plays a key role in workflow management, training, and providing exceptional service in a specialty pharmacy environment. Key Responsibilities

- Mentor and guide Patient Care Coordinators and assist in training and development
- Troubleshoot and resolve escalated issues related to referrals and patient/provider concerns
- Monitor phones, emails, and workflow; balance independent and team-based tasks
- Obtain lab results to support Clinical Pharmacists with care planning
- Handle complex and priority payer/pharma accounts
- Contact new patients to introduce pharmacy services and gather necessary documentation
- Maintain accurate patient records and ensure timely medication ordering
- Review inventory and coordinate medication deliveries
- Process patient payments and communicate co-pay/past due balances
- Document all communications with stakeholders (pharmacy personnel, physicians, patients, etc.)
- Ensure pharmacy records are complete and accurate
- Assist in developing and implementing training materials and quality audits
- Supervise daily operations and assign team workflow effectively
- High school diploma or equivalent
- 3+ years of experience in pharmacy technician roles, call center operations, or customer service
- Strong leadership and customer service skills
- Healthcare and specialty pharmacy experience (preferred)
- Call center and insurance background strongly preferred
- Infusion center experience is a plus
- Current Pharmacy Technician Certification (preferred)
- Foundation Care Only: Must complete Missouri Registered Pharmacy Tech application on Day 1 and obtain a non-restricted license within 30 days OR hold an active Missouri license
