Care Management Support Coordinator II
Position Title: Care Management Support Coordinator II (Remote )
Locations: REMOTE - WA, OR, NV OR AZ
Anticipated Start Date: 03/21
Expected Duration: 3 Monnths
Job Type: Contract with possible extension
Timings: 8-5 Monday-Friday PST-No flexibility in schedule
Pay Rate: $25.00/hr. on W2.

Job Description:
Position Overview:
The Care Management Outreach Specialist plays a key role in supporting the Care Management Team for the California market. This is a high-volume, fast-paced call center role focused on engaging existing members, educating them on care management programs, and enrolling them in relevant healthcare services. Success in this role is measured by outreach effectiveness, enrollment conversions, and overall production metrics.
Key Responsibilities:
- Conduct outbound calls to existing members, discussing their healthcare needs and encouraging program enrollment.
- Educate members on the benefits of the Care Management Program and guide them through the enrollment process.
- Maintain a minimum of 60 calls per day in a dynamic, fast-paced environment.
- Utilize sales techniques to engage members, overcome objections, and increase program participation.
- Accurately document call outcomes and member information in TrueCare and other internal systems.
- Navigate multiple systems efficiently while handling member inquiries and updating records.
- Generate basic reports in Excel as needed for tracking performance and enrollment trends.
- Adapt to evolving business needs, handling different types of member outreach calls as required.
- Ensure compliance with healthcare regulations, company policies, and call center standards.
Qualifications & Experience:
- Healthcare experience required (familiarity with care management programs preferred).
- Call center experience required (outbound call experience preferred).
- Sales experience highly preferred-ability to confidently pitch and promote program benefits to members.
- Strong data entry skills with the ability to work across multiple systems simultaneously.
- Basic Excel proficiency for tracking and reporting purposes.
- Ability to thrive in a target-driven, performance-based environment.
- Flexibility to adapt to evolving business needs and handle different call types as required.

Education Requirement:
- High school diploma or equivalent
- Proficiency in Excel and MS Office
- Outbound Call Volume: Minimum 60 calls per day to existing members.
- Member Enrollment: Success measured by number of members enrolled in the Care Management Program.
- Quality & Compliance: Adherence to call scripts, regulatory guidelines, and company protocols.
- Productivity Tracking: Performance monitored based on call volume, engagement, and successful enrollments.
