Anticipated Start Date: 10/03
Expected Duration: 6 Months
Job Type: Contract with possible extension
Shift: Monday-Friday, 8:00 a.m.-6:00 p.m. (based on an 8-hour shift).
Pay Rate $17.86/hr on W2.
Need: Customer service experience with expertise in care coordination, navigation, and community resource awareness.
Note : Must Pass Typing Test (55WPM) AND Computer Literacy Test.

Job Summary

Key Duties and Responsibilities
- Assist members in connecting with community and care resources in a cost-effective manner.
- Coordinate community outreach resources and promote awareness of available services.
- Serve as a point of contact for member inquiries regarding community and care resources.
- Support member-related correspondence and develop educational materials to facilitate successful community connections.
- Document and maintain information on community resources, ensuring compliance with health plan requirements.
- Provide assistance to the clinical team of nurses and social workers through outreach, community education, informal guidance, and member support.
- Conduct non-clinical general health assessments to refer members to appropriate care/services, resolve concerns, and collect information for medical providers and staff.
- Perform non-medical assessments such as home safety, community/environmental resources, transportation, and employment to support referrals and gather relevant information.
- Conduct telephonic and/or in-person outreach to locate individuals and families who are difficult to reach.
- Visit individual homes and/or community organizations as needed.
- Demonstrate working knowledge of Social Determinants of Health (SDOH) barriers.
- Perform other duties as assigned.
- Comply with all policies and organizational standards.
Minimum Qualifications
Education and Experience
- High School diploma or GED required.
- 1-2 years of related experience required.
- Monday-Friday, 8:00 a.m.-6:00 p.m. (based on an 8-hour shift).
Story Behind the Need
- Purpose of the Team: Support underserved populations in managing their health and provide education on inappropriate ER utilization (Ambetter and possibly Medicare populations).
- Team Culture/Environment: Fast-paced, collaborative health team culture with goals including increasing member satisfaction, closing care gaps, and reducing inappropriate utilization (e.g., ER visits).
- Hiring Initiative: This is not part of a larger hiring initiative.
Typical Day in the Role
- Perform face-to-face care management functions.
- Network with community-based organizations.
- Serve as a liaison between the community and the health plan as a Community Health Service Representative (CHSR).
- Meet performance expectations including monthly compliance and quality case audits.
