BCforward is currently seeking a highly motivated Care Management Support Coordinator II for an opportunity Remote!

Job Title: Care Management Support Coordinator II

Work Location: Remote (West states preferred: WA, OR, NV, AZ)

Expected Duration: 3 Months

Target Start Date: February 20, 2025

Work Shift: 8:00 AM - 5:00 PM, Monday - Friday PST

Payrate : $25/hr

Position Purpose

The Care Management Support Coordinator II supports administrative care management activities including outreach, inbound call handling, and scheduling services. This role serves as a point of contact for members, providers, and internal teams to resolve issues and document member records in compliance with state and regulatory guidelines.

Key Responsibilities

Outreach & Member Support:

  • Perform outbound calls to members to support care plan next steps, provide community or health plan resources, and assist with questions or concerns related to scheduling.
  • Guide members through ongoing education for both the member and provider throughout the care/service.
  • Connect members with other health plan and community resources to ensure high-quality customer care.
  • Apply knowledge of assigned health plan(s) to answer inquiries and resolve issues.

Documentation & Record Keeping:

  • Document and maintain non-clinical member records to comply with state regulations and internal policies.
  • Ensure documentation is accurate and timely to meet both regulatory and operational requirements.

Member Onboarding & Administration:

  • Assist with member onboarding, including sending welcome letters, related correspondence, and program educational materials.
  • Provide day-to-day administrative support to facilitate positive member/provider relationships.

Social Determinants of Health (SDOH):

  • Make referrals to address SDOH needs, leveraging local benefits and resources.

Other Duties:

  • Perform other related duties as assigned by supervisor.
  • Comply with all policies and standards.

Education/Experience

Required:

  • High School diploma or GED.
  • 1-2 years of experience in a related field.

Preferred:

  • At least 2 years of healthcare experience.
  • Call center or customer service experience.
  • Sales experience is highly preferred.
  • Bilingual in Spanish is a plus.

Required Skills/Experience

  • Ability to manage a high volume of calls in a fast-paced environment.
  • Strong communication skills and ability to engage members in discussion regarding their care.
  • Basic data entry skills and proficiency in navigating multiple systems.
  • Experience with Microsoft Excel for basic reporting.
  • Adaptability to change and flexible with types of calls handled.
  • Strong attention to detail in maintaining accurate member records.

Preferred Skills/Experience

  • Previous experience in outbound sales, customer service, or healthcare-related roles.
  • Ability to maintain a high level of productivity, making a minimum of 60 calls per day.
  • Familiarity with True Care or similar systems.
  • Knowledge of health plan activities and resources.
