Care Management Support Coordinator II
Position Title: Care Management Support Coordinator II (Remote - KY) Locations: Remote - Kentucky
Anticipated Start Date: 07/07
Expected Duration: 6 Months
Job Type: Contract with possible extension
Shift: Typically 8:00 AM-5:00 PM, 8:30 AM-5:30 PM, or 9:00 AM-6:00 PM; flexible with later start times if needed.
Pay Rate $22.00/hr. on W2
Need: Experience in healthcare or community settings with knowledge of medical terminology, screenings, MCOs, and strong basic computer skills.
Job Description:
Day-to-Day Responsibilities & Project Description (Outside of Workday JD):
The role involves extracting member data from internal systems and entering it into spreadsheets. The primary responsibility is to conduct outreach to members to help close care gaps, particularly by assisting them with scheduling primary care provider (PCP) appointments and locating a PCP if needed. The position also includes helping members access resources related to social determinants of health (SDOH). All interactions and outcomes must be accurately documented across various systems.
Performance Expectations/Metrics:
- Complete a weekly call goal of 250 calls
- Manage a daily panel of 50-60 members
- Maintain required monthly audit scores based on quality and compliance standards
- Introduction to the company and team
- Initial setup of laptop and access to required systems
- Orientation calls with HR, corporate team, and training staff
- Begin onboarding and shadowing process
- Registration Clerk
- Medical/Patient Care Assistant
- Mental Health Care Professional
- Customer Service Representative
- Licensed Practical Nurse (LPN)
- Community Health Worker
- Demonstrated long-term tenure in previous positions
- Experience in a medical office or healthcare setting
- Familiarity with medical terminology
- Experience with community-based organizations
- Basic computer proficiency
- Managed Care Organization (MCO) experience
- Understanding of common medical screenings
- High School Diploma
- Medical training, certification, or background in mental health
Key Responsibilities:
- Conduct outreach to members via phone to support care plan activities, address questions, and provide ongoing education to both members and providers.
- Assist members in accessing health plan and community resources to promote high-quality care and service.
- Utilize working knowledge of the assigned health plan(s) to assist members and providers effectively.
- Respond to member and provider inquiries, requests, and concerns, including clarification of care plan procedures and protocols.
- Support member onboarding by sending welcome letters, correspondence, and program-related educational materials.
- Maintain and document non-clinical member records in alignment with regulatory and organizational standards.
- Make appropriate referrals to address Social Determinants of Health (SDOH) by leveraging knowledge of local resources and benefits.
- Perform additional duties as assigned.
- Ensure full compliance with company policies, procedures, and regulatory standards.
Education & Experience Requirements:
- Required: High School Diploma or GED
- Experience: 1-2 years of related experience in a healthcare, customer service, or administrative support role
