Care Coordinator II
Position Title: Care Coordinator II (Remote - Spanish) Locations: Remote (with in-office requirement whenever needed) - Tampa, FL 33634
Anticipated Start Date: 5/26
Expected Duration: 6 Months
Job Type: Contract with possible extension
Shift: Monday - Friday 9:00 a.m. - 6:00 p.m. EST
Pay Rate $25.00/hr. on W2
Need: Minimum 2+ years in healthcare or managed care with high-volume call experience, tech proficiency (Excel, Outlook, Word, MS Teams); bilingual (Spanish/Creole) and prior remote work experience preferred.

Job Description:
Position Purpose
As a Care Management Support Specialist, you will play a critical role in ensuring that healthcare services are efficiently delivered to members, supporting care managers, and promoting continuity of care. You will engage with members through outbound phone calls or home visits, coordinating services and educating members on their care plans and available resources.
Key Responsibilities
- Conduct outreach via phone or home visits to engage members and discuss their care/service plans, next steps, available resources, and education needs.
- Coordinate care activities with healthcare and community providers, partners, caregivers, and internal teams.
- Handle member and provider inquiries and concerns related to care plans, and escalate as needed.
- Communicate with care managers, practitioners, and internal stakeholders to ensure continuity of care.
- Assist in service assessments/screenings and accurately document member needs.
- Maintain up-to-date member records per regulatory and organizational guidelines.
- Identify member needs and refer to appropriate internal or community-based programs.
- Provide clear education on member benefits and available healthcare resources.
- Submit daily reports summarizing outreach efforts and productivity.
- Perform other duties as assigned, in alignment with company policies and standards.
To support increased volume of care coordination cases and enhance member outreach compliance.

Typical Day in the Role
- Conduct 50-75 outbound member calls per day
- Make 3 contact attempts per member (Day 1, 2, and 3), followed by provider outreach if contact fails
- Investigate and research member contact information, case details, and provider coordination
- Submit daily productivity reports via Outlook
- Primarily remote, but MUST report in-person to designated office if remote work is interrupted (e.g., internet outage)
- May be assigned stretch projects depending on workload

Qualifications
Education & Certification
- Required: High School diploma or GED
- Preferred: Additional education should be listed and not omitted (e.g., Associate's, Bachelor's)
- Required:
- 2+ years in healthcare or managed care (non-negotiable)
- High-volume inbound/outbound calling experience
- Proficiency with basic technology (Excel, Outlook, Word, MS Teams)
- 2+ years in healthcare or managed care (non-negotiable)
- Preferred:
- Bilingual (Spanish or Haitian Creole)
- Previous remote work experience
- Bilingual (Spanish or Haitian Creole)
- Lack of clear recent work history or missing contract experience (especially with Centene)
- Employment gaps of 6-8 months without explanation
- Inaccurate or incomplete education history
Performance Expectations
- Ability to multi-task (e.g., making calls while taking detailed notes)
- Strong attention to professional conduct and appearance on remote calls
- Must have a quiet, professional home office setup to handle confidential health information
