Job Description:
Position Purpose:
The Care Management Support Coordinator plays a vital role in assisting with administrative care management activities, including conducting outreach, handling inbound calls, and coordinating services. This position serves as a primary point of contact for members, providers, and internal teams, ensuring seamless communication, issue resolution, and accurate documentation in compliance with state and regulatory guidelines.
Fast-paced environment with a moderate call volume. As the frontline representative for the Care Management department, the temp will engage with numerous individuals daily, handling a variety of unique conversations.

- Process 10 Care Management (CM) referrals daily
- Maintain a call volume of 50+ calls per day

- Strong communication and problem-solving abilities
Education & Experience:
- High school diploma or GED required.
- 1-2 years of related experience in healthcare, customer service, or administrative support.
- Conduct outreach to members via phone to support care plan next steps, address questions or concerns, and provide ongoing education for both members and providers.
- Assist members in accessing health plan and community resources, ensuring they receive high-quality support and services.
- Apply knowledge of health plan activities and available resources to effectively guide members and providers.
- Serve as the first point of contact for member and provider inquiries, explaining care plan procedures and protocols.
- Support member onboarding by managing administrative tasks, including sending welcome letters, program materials, and other relevant correspondence.
- Maintain and update non-clinical member records in compliance with state and regulatory standards, providing documentation to providers as needed.
- Identify and refer members to local benefits and resources to address Social Determinants of Health (SDOH) needs.
- Perform other duties as assigned.
- Adhere to all company policies and regulatory standards.
