Job Title: Care Management Support Coordinator II
Locations: Remote Target Start Date: 28 April 2025Expected Duration: 6 Months Pay Rate $24.00/hr. on W2 Position Purpose:The Care Management Support Coordinator II supports administrative care management activities, including performing outreach, answering inbound calls, and scheduling services. This position serves as a point of contact for members, providers, and staff to resolve issues and document member records in accordance with state and regulatory guidelines. Education & Experience Requirements:

Job Title: Care Management Support Coordinator II

Education & Experience Requirements:
- Education:
- High School diploma or GED required.
- Experience:
- 1-2 years of related experience preferred, particularly in a call center, medical insurance, or customer service setting.
- Member Outreach & Support:
- Provide outreach to members via phone to assist with care plan next steps, health plan/community resources, and scheduling-related questions.
- Educate members and providers regarding the care process and ongoing educational needs.
- Assist members in connecting with additional health plan and community resources to ensure they are receiving comprehensive, high-quality care.
- Documentation & Records Management:
- Document and maintain non-clinical member records, ensuring compliance with state and regulatory requirements.
- Provide members' information to providers as needed to facilitate care.
- Service Coordination:
- Assist members with understanding their care plan procedures and protocols.
- Support member onboarding by sending welcome letters and educational materials to facilitate a successful member-provider relationship.
- Health Plan & Community Resource Knowledge:
- Maintain knowledge of available benefits and local resources, making appropriate referrals to address Social Determinants of Health (SDOH) needs.
- Administrative Duties:
- Perform various administrative tasks, such as coordinating scheduling for peer-to-peer authorizations and handling member/provider inquiries efficiently.
- Other Duties:
- Complete other tasks as assigned by management.
- Comply with company policies and regulatory standards.
- Work Location:
- Nationally sourced (Remote)
- Work Hours (EST):
- 8:00 AM - 5:00 PM (Opener)
- 10:00 AM - 7:00 PM (Mid)
- 11:00 AM - 8:00 PM (Closer)
- Must-Have Skills:
- Call center experience
- Medical insurance experience
- Strong customer service skills
- Excellent communication and problem-solving abilities
- Ability to adapt quickly to new processes and systems
- Nice-to-Have Skills:
- Trucare experience
- Experience with phone systems
