BCforward is currently seeking a highly motivated Care Management Support Coordinator II for an opportunity in Remote (WA, Oregon , NV or AZ)!
Position Title: Care Management Support Coordinator II
Location: Remote (WA, Oregon , NV or AZ)

This role supports administrative care management activities such as performing outreach, answering inbound calls, and scheduling services. The individual will serve as a point of contact for members, providers, and staff to resolve issues and ensure compliance with state and regulatory guidelines. The role includes maintaining member records, addressing inquiries, and providing resources for members and providers.Key Responsibilities:
- Perform outreach to members via phone to discuss care plans, community or health plan resources, and any concerns related to scheduling or ongoing education.
- Assist members in connecting to health plan and community resources to ensure high-quality care and service.
- Serve as front-line support for member and provider inquiries, explaining care plan procedures and protocols.
- Support member onboarding by sending welcome letters, related correspondence, and program educational materials.
- Document and maintain non-clinical member records in accordance with state and regulatory requirements.
- Address Social Determinants of Health (SDOH) by making relevant referrals.
- Provide assistance in a fast-paced call center environment, including outbound calls to members to enroll them in Care Management programs.
- Maintain productivity metrics including the number of calls made and members enrolled in the program.
- Perform data entry into True Care and navigate multiple systems.
- High School diploma or GED.
- 1-2 years of experience in a related field, particularly in call center, customer service, or sales roles.
- Healthcare experience required.
- Sales experience is highly preferred.
- Bilingual in Spanish is a plus.
- Strong communication skills to handle member and provider inquiries.
- Ability to adapt to changes in work processes and call types.
- Ability to make outbound sales calls and enroll members into care management programs.
- Proficient in navigating systems and performing data entry.
- Experience in Excel for basic reporting.
- Ability to work in a fast-paced, production-based environment with specific call volume goals (minimum of 60 calls per day).
