BCforward is currently seeking a highly motivated Care Manager (RN) for an opportunity in Remote-IN!

Pay Range: [$45] - [$48.09]Please note that actual compensation may vary within this range due to factors such as location, experience, and job responsibilities, and does not encompass additional non-standard compensation (e.g., benefits, paid time off, per diem, etc.).
Requirements:- 2-4 years of related experience, with OB experience (prenatal, postpartum, labor/delivery, mother-baby, or NICU) strongly preferred.
- Active Registered Nurse License in Indiana or Compact State Licensure.
- Evaluate member needs, identify barriers to care, and address Social Determinants of Health.
- Develop, monitor, and revise member-centered care plans in collaboration with providers and caregivers.
- Provide telephonic and digital outreach to engage members and assess health status.
- Coordinate services and ensure timely access to appropriate healthcare providers and community resources.
- Educate members and caregivers on healthcare conditions, benefits, and self-management strategies.
- Maintain accurate documentation to ensure compliance with state, federal, and organizational regulations.
- Participate in interdisciplinary care rounds and provide performance feedback to leadership.
- Meet a caseload target of 50 members with a minimum of 5 new enrollments per month.
- Ensure quality charting that meets NCQA standards (monthly audits ≥94% compliance).
- Degree from an accredited School of Nursing or a Bachelor's degree in Nursing required.
- 2-4 years of related experience, with OB experience (prenatal, postpartum, labor/delivery, mother-baby, or NICU) strongly preferred.
- Required: Active Registered Nurse License in Indiana or Compact State Licensure.
