Care Management Support Coordinator II
BCForward is looking for Care Management Support Coordinator II- Remote
Position Title: Care Management Support Coordinator II
Location: Remote
Expected Duration: 06+ months (Contract)
Pay Rate: $22.00/hr on W2
Job Type: Full-Time (40 hours/week) | Contract | Remote
Work Schedule:
- Available to work 7 am - 4 pm PST / 8 am - 5 pm PST.
- Initial schedule: 7 am - 4 pm PST, Monday-Friday.
- May transition to 8 am - 5 pm PST including weekends as needed.
- Occasional weekend work may be required.
- Overtime not required but may be requested.
- Breaks and lunches are scheduled due to call center environment.
- Work from home in a fast-paced environment, collaborating with clinical teams to improve provider experience and member service delivery.
Position Purpose:
Supports administrative care management activities including performing outreach, answering inbound calls, and scheduling services. Serves as a point of contact to members, providers, and staff to resolve issues and documents member records in accordance with current state and regulatory guidelines.
Key Responsibilities:
- Provide outreach to members via phone to support with care plan next steps, community or health plan resources, scheduling, and ongoing education.
- Support members by connecting them to health plan and community resources to ensure high-quality customer care.
- Apply working knowledge of assigned health plan(s) activities and resources.
- Serve as front-line support for member/provider inquiries, requests, or concerns, including care plan procedures and protocols.
- Assist with member onboarding and administrative duties including sending welcome letters, correspondence, and educational materials.
- Document and maintain non-clinical member records in compliance with state and regulatory requirements.
- Refer members to local resources to address Social Determinants of Health (SDOH) needs.
- Perform other duties as assigned.
- Comply with all policies and standards.
Must-Have Hard Skills (Stack-Ranked):
- Call Center Experience - Intermediate to Advanced
- Healthcare Background - Basic to Intermediate
- Strong Verbal and Written Communication Skills - Advanced
- Scheduling experience
- Microsoft Outlook proficiency
- Previous peer-to-peer (P2P) experience
- Critical thinking
- Ability to take feedback well
- Adaptability
Education/Certification:
- Required: High School diploma or GED
- Preferred: College education
