Job Details:
Position: Care Management Support Coordinator II
Location: Remote - Candidates must be located in EST or CST time zones and must not reside in Florida.
Work Schedule: 8-5. 8:30-5:30, or 9-6
Duration: 3 Months (possibility for extent) - Contract to Hire
Work Type: Remote
Job Summary:
- Supports administrative care management activities, including performing outreach, answering inbound calls, and scheduling services. Serves as a point of contact for members, providers, and staff to resolve issues and documents member records in accordance with current state and regulatory guidelines.
Education/Experience:
- Requires a high school diploma or GED.
- Requires 1-2 years of related experience.
Responsibilities:
- Provides outreach to members via phone to support care plan next steps, community or health plan resources, and to address questions or concerns related to scheduling and ongoing education for both the member and provider.
- Supports members in connecting with health plan and community resources to ensure they receive high-quality customer care/service.
- Applies working knowledge of assigned health plan(s) activities and resources.
- Acts as front-line support for various member and/or provider inquiries, requests, or concerns, including explaining care plan procedures and protocols.
- Supports member onboarding and performs administrative duties, including sending welcome letters, related correspondence, and program educational materials to facilitate successful member/provider relationships.
- Documents and maintains non-clinical member records to ensure adherence to standards of practice, state regulations, and organizational policies; provides documentation to providers as needed.
- Utilizes knowledge of local benefits and resources to make referrals addressing Social Determinants of Health (SDOH) needs.
- Performs other duties as assigned.
- Complies with all policies and standards.
