BC Forward is looking for Care Manager (RN) - Remote (IN)

Care Manager (RN)

Location : Remote (IN)
Duration : 11 months of contract

Pay Rate: $45.00 per hour

Need: Graduation from an accredited School of Nursing with 2-4 years of relevant clinical or care management experience required; Bachelor's degree in Nursing. Active Indiana State RN License OR Compact License. Required: OB experience (Prenatal, Postpartum, Labor/Delivery, Mother-Baby, or NICU).

Position Purpose:

  • Develops, assesses, and facilitates complex care management activities for primarily physical needs members to provide high-quality, cost-effective healthcare outcomes, including personalized care plans and education for members and their families.

Education/Experience:

  • Requires a Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing and 2 - 4 years of related experience.

License/Certification:

  • RN - Registered Nurse - State Licensure and/or Compact State Licensure required.

Responsibilities:

  • Evaluates member needs, barriers to care, and social determinants of health needs, focusing on the member's identified priorities.
  • Develops care plans and collaborates with providers to address unmet needs.
  • Identifies care barriers and provides care management interventions.
  • Coordinates care with providers, members, and families/caregivers to ensure quality care delivery.
  • Provides ongoing follow-up and updates care plans to meet the member's goals.
  • Connects members with local resources for services such as employment, housing, independent living, etc.
  • May perform telephonic, digital, or home outreach to assess needs and coordinate care.
  • Collects, documents, and maintains member information and care management activities to ensure compliance with regulations.
  • Provides education to members and families on disease processes, care gaps, healthcare instructions, and benefits.
  • Offers feedback to leadership on opportunities to enhance care and quality delivery.

Typical Day in the Role:

  • Enrolls members, assesses needs, finds resources, closes care gaps, manages caseload, charts to NCQA standards, ensures timely charting, collaborates with other care managers.
  • Performance expectations include enrolling members, managing caseloads, and keeping up-to-date with training and quality standards.
  • This role is unique because it offers flexibility to work from home, and members make a meaningful impact while being part of a supportive, well-established team.

Candidate Requirements:

Education/Certification:

  • Associates or Bachelor's in Nursing.

Licensure:

  • Required: Current, active unrestricted RN license in the state of Indiana.

Years of Experience Required:

  • Minimum of 3 years of clinical experience or an equivalent combination of education and experience.

Preferred Qualifications:

  • OB nursing experience.
  • Case management experience.
  • Managed care experience.
  • Maternal/Child health experience such as OB, Postpartum, pediatric, or NICU.
