BCforward is currently seeking a highly motivated Care Management Support Coordinator II for an opportunity in Remote (CST)!
Position Title: Care Management Support Coordinator II
Location: Remote (CST)

Pay Range: [$20.00] - [$23.00]Please note that actual compensation may vary within this range due to factors such as location, experience, and job responsibilities, and does not encompass additional non-standard compensation (e.g., benefits, paid time off, per diem, etc.).

The Care Management Support Coordinator II plays a vital role in supporting administrative care management activities. This includes conducting outreach, managing inbound calls, scheduling services, and serving as a point of contact for members, providers, and staff. The position ensures the documentation of member records complies with state and regulatory guidelines. Key Responsibilities:
- Perform outreach to members to assist with care plan next steps, address questions, and provide education on available resources.
- Support members by connecting them to health plan and community resources to ensure quality customer service.
- Serve as front-line support for member and provider inquiries, including explaining care plan procedures and protocols.
- Facilitate member onboarding by sending welcome letters, educational materials, and related correspondence to strengthen member/provider relationships.
- Maintain non-clinical member records to meet state and regulatory requirements.
- Address Social Determinants of Health (SDOH) needs by referring members to local resources.
- Meet a daily target of 60-70 calls to achieve performance goals.
- Adhere to company policies and standards while performing other assigned duties.
- Education: High school diploma or GED required.
- Experience: 2-3 years of experience in a managed care, medical office, or facility setting demonstrating medical administration duties.
- Experience in high-call environments such as call centers, 911 dispatch, or managed care/Medicaid/Medicare brokerages is preferred.
- Strong customer service and communication skills.
- Proficiency in Microsoft Office applications.
- Ability to manage a high call volume efficiently (60-70 calls daily).
- Previous sales or enrollment specialist experience is a plus.
- Detail-oriented with the ability to document and maintain accurate records.
- Maintain an average of 60-70 calls daily.
- Deliver high-quality customer service while meeting metric-driven performance goals.
