BC Forward is looking for Care Management Support Coordinator II (REMOTE)

Position Title: Care Management Support Coordinator II (REMOTE)

Locations: REMOTE - EST/CST

Anticipated Start Date: 03/10

Expected Duration: 4 Months

Job Type: Contract with possible extension

Shift: 9am-6pm EST/8am-5pm CST

Pay Rate $20/hr. on W2

Need: Min 2 yrs of Healthcare/Managed Care Experience with High Volume Outbound calling.


Job Description:
Role Overview:
This position involves supporting the medical and psychological aspects of utilization and coordinated care. The candidate will be responsible for conducting outreach to assigned members, maintaining accurate records, and ensuring consistent communication with supervisors.
Key Responsibilities:
  • Assist in tasks related to medical and psychological utilization and coordinated care.
  • Conduct daily outreach via telephone to assigned members.
  • Maintain and update an Excel spreadsheet with member assignments.
  • Ensure consistent and timely communication with the supervisor.
  • Accurately document each phone call in the Virtual Health system.
Performance Expectations & Metrics:
  • Conduct 80-120 outbound calls per day to the Medicare population.
  • Demonstrate a strong understanding of medical terminology.
  • Maintain 100% compliance with assigned member outreach.
  • Achieve an engagement rate of 80% or higher each month.
  • Ensure an audit score of at least 90%.
  • Complete outreach to 600 members per month (30 new members assigned daily).
  • A dedicated, hardwired home internet connection is required to comply with HIPAA regulations (include this in the resume).
  • Time off is not permitted within the first 90 days unless pre-approved during the interview process.
First-Day Overview:
  • The candidate will begin with an IT onboarding session to ensure all equipment and systems are fully operational.
  • Following IT setup, they will complete required training modules assigned by the supervisor.
  • The first week will be dedicated to training, covering company policies, compliance requirements, and role-specific tasks.
  • Once training is complete, hands-on instruction and role-specific job training will commence.
Required Skills & Experience:

  • 2-3 years of experience in a managed care setting, medical office, or facility with demonstrated medical administration duties.
  • Strong knowledge of customer service, utilization review, or claims processing within a managed care environment.
  • Proficiency in operating office equipment, including personal computers.

  • Preferred Skills & Experience:

  • Strong communication skills.
  • Knowledge of medical terminology.
  • Excellent time management and organizational skills.

  • Education Requirements:
    • Required: High school diploma or equivalent.
    • Preferred: Additional relevant education or certifications.
    Software Skills Required:
    • Basic proficiency in Microsoft Office.
