Position Summary
The Care Management Support Coordinator II will support administrative care management activities, focusing on outreach to members, scheduling services, and resolving inquiries. This role involves making outbound calls in a fast-paced call center environment, assisting with care plan education, and enrolling members into care management programs. The coordinator will also maintain compliance with state and regulatory requirements and provide excellent service to members and providers.

The coordinator will also maintain compliance with state and regulatory requirements and provide excellent service to members and providers.
Key Responsibilities

Member Outreach:
- Make a minimum of 60 outbound calls daily to engage members about care management programs.
- Educate members on care plans, benefits, and available resources.
- Encourage and enroll members in care management programs.

Customer Support:
- Serve as a point of contact for member and provider inquiries and issues.
- Assist with onboarding, sending welcome letters, and distributing educational materials.

Data Management:
- Document and maintain non-clinical member records.
- Ensure compliance with policies, standards, and state/regulatory guidelines.
- Utilize systems like True Care and Excel for data entry and reporting.

Collaboration:
- Support internal teams by providing member information and making referrals for Social Determinants of Health (SDOH) needs.
- Adapt to changing business needs and handle various call types as required
Qualifications
- High School diploma or GED required.
- 1-2 years of fast-paced call center experience required.
- At least 2 years of healthcare experience required.
- Sales experience highly preferred; ability to make persuasive pitches to members.
- Bilingual in Spanish is a plus.
- Proficiency in MS Office (Excel preferred for basic reporting).
- Strong ability to navigate multiple systems efficiently.
- Excellent communication and interpersonal skills.
- Proven ability to adapt to changing environments and priorities.
- Achieve daily production goals (minimum 60 calls per day).
- Successfully enroll members into care management programs.
- Maintain high-quality customer service standards and adaptability.
