Care Management Support Coordinator II
Position Title: Care Management Support Coordinator II (REMOTE - DE ) Locations: REMOTE - DelawareAnticipated Start Date: 03/10Expected Duration: 3 MonthsJob Type: Contract with possible extension Shift: 8:00 AM to 5:00 PM ESTPay Rate $22/hr. on W2 Need: Min 2 yrs of Healthcare/Managed Care Experience
Job Description:
Position Purpose:
Assist in activities related to the medical and psychosocial aspects of utilization and coordinated care. This role focuses on member outreach, appointment coordination, community resource connections, and provider communication to ensure comprehensive support for members.
Key Responsibilities:
- Conduct member outreach calls to assist with care coordination, appointment scheduling, and follow-up on social determinants of health needs.
- Facilitate appointment linkage and verify scheduled medical visits to ensure compliance with treatment plans.
- Research and resolve claims inquiries related to departmental responsibilities.
- Assess and monitor the inpatient census to track members' hospitalization status.
- Screen for eligibility and benefits, identifying members without a Primary Care Provider (PCP) and referring them to Member Services.
- Prioritize and screen members for case management (CM) assessments.
- Perform transition of care duties, including coordinating with attending physicians, home health agencies, equipment vendors, and other medical providers.
- Connect members with appropriate community-based organizations to address their healthcare and social needs.
- Represent the organization in marketing and outreach meetings as required.
- Generate and mail routine case management letters and educational materials.
- Accurately enter assessments and authorizations into the system.
Required Skills & Experience:

- Strong communication and problem-solving skills
- Familiarity with case management or utilization review processes
- Ability to work in a fast-paced healthcare setting
Education Requirements:
- Minimum: High school diploma or equivalent
- Experience: 2-3 years in a managed care setting, medical office, or healthcare facility with demonstrated medical administrative duties
Software Skills Required:
- Microsoft Outlook (email)
- Microsoft Teams
- Microsoft Office Suite (Word, Excel, PowerPoint)
- Spreadsheets
Certifications & Testing:
- Required Certifications: None
- Required Training: University training programs
- Successfully complete a minimum of 40 calls per day to assigned members.
- Meet weekly outreach and engagement targets as set by the department.
Onboarding & Training:
- First-day orientation includes access to Centene University and an introduction to company policies and procedures.
- The role requires 4 to 6 weeks of structured training, including hands-on training with the hiring manager and department-specific instruction.
Preferred Background & Experience:
Education:
- High school diploma or equivalent required.
- 2-3 years of experience in a managed care setting, medical office, or healthcare facility with responsibilities related to medical administration.
- Strong knowledge of customer service, utilization review, or claims processing in a managed care environment.
- Familiarity with medical terminology and healthcare processes.
- Proficiency in using office equipment and systems, including Microsoft Office and electronic health record (EHR) platforms.
