Position Purpose:
The Care Management Coordinator supports the senior care management team by ensuring seamless service delivery to members through healthcare providers and community partners. This role serves as a liaison between care managers, providers, and members to coordinate care effectively. The coordinator engages with members via phone outreach or home visits to facilitate continuity of care and enhance member satisfaction.
The Absolute Total Care Quality Improvement Team fosters a close-knit, collaborative atmosphere in a remote setting. This team conducts outbound calls to members, providing health reminders and encouraging them to schedule essential appointments.
The workday runs from 8:00 AM to 5:00 PM and includes two paid 20-minute breaks and one unpaid 60-minute lunch break. Communication is primarily conducted via email and chat, fostering a close-knit, collaborative atmosphere in a remote setting.
The team is driven by a passion for people, engagement, and improving quality of life. We continuously adapt to and learn about healthcare changes to enhance our outreach efforts and deliver better outcomes.

Qualifications & Expectations
Must-Haves:
- Minimum of 2+ years of call center experience, preferably in healthcare.
- Experience with HEDIS, STARS, CAHPS, NCQA, Medicare, and/or Medicaid.
- Distractions during the interview.
- Lack of professionalism or not being camera-ready.
- Inability to answer questions effectively.
- Frequent job hopping.
- Other concerns (TBD).
- Make 70 calls per day (10 calls per productive hour).
- Maintain a 99.5% quality score.
- Adhere to attendance guidelines.
Education & Experience:
- High School diploma or GED required.
- 2 - 4 years of relevant experience in healthcare, social services, or a related field.
- For the Illinois Youth Care Contract: A Bachelor's degree in nursing, social sciences, social work, or a related field, along with one (1) year of supervised clinical experience in a human services field, is required.
- Specific licensure or certification may be required based on state regulations and contractual obligations.
- Conduct outreach to members via phone or home visits to discuss care plans, service plans, next steps, available resources, and address any questions or concerns.
- Coordinate care activities based on individual care/service plans, working closely with healthcare and community providers, members, and caregivers to ensure effective service delivery.
- Serve as a point of contact for members and providers, addressing inquiries, requests, and concerns related to care plans.
- Develop in-depth knowledge of care management services and assist in resolving complex or escalated issues.
- Facilitate communication between care managers, practitioners, and other stakeholders to support member services and ensure continuity of care.
- Conduct service assessments and screenings for members with complex needs, documenting their care requirements in accordance with state and regulatory guidelines.
- Maintain accurate and up-to-date member records and distribute them to providers as needed.
- Support the care management team by triaging, prioritizing, and escalating complex requests to management when necessary.
- Ensure compliance with all contractual, regulatory, and organizational policies and standards.
- Identify member needs and provide appropriate referrals to care managers, community-based organizations, and disease management programs.
- Educate members on benefits, available resources, and care options.
- Assist in training and development activities as needed.
- Perform additional duties as assigned.
