BCForward is looking for Care Management Support Coordinator II- Remote PST
Position Title: Care Management Support Coordinator II
Location: Remote PST
Position Purpose:
- The Care Management Support Coordinator II supports administrative care management activities, including outreach to members, handling inbound calls, and scheduling services. This role serves as a point of contact for members, providers, and staff to resolve issues and document member records in accordance with state and regulatory guidelines.
Education/Experience:
- High School diploma or GED required.
- 1-2 years of related experience preferred.
- Previous experience in a fast-paced call center is required, with a strong preference for sales experience.
Key Responsibilities:
- Conduct outreach to members via phone to assist with care plans, community resources, and any scheduling concerns.
- Provide support to connect members with health plan and community resources to ensure high-quality customer service.
- Serve as the primary support for member and provider inquiries, explaining care plan procedures and protocols.
- Assist with member onboarding, including sending welcome letters and educational materials to facilitate successful member/provider relationships.
- Document and maintain non-clinical member records, ensuring compliance with state and regulatory standards.
- Understand existing benefits and resources to address Social Determinants of Health (SDOH) needs.
Required Skills:
- Proficient in Microsoft Excel and other MS Office applications.
- Ability to navigate multiple systems and perform data entry accurately.
- Strong communication skills, both oral and written.
- Adaptability to change in job functions and responsibilities.
Preferred Qualifications:
- Experience in healthcare is required.
- Bilingual in Spanish is a plus.
- Prior outbound sales experience is highly preferred.
