BCforward is currently seeking a highly motivated Care Management Support Coordinator
Position Title: Care Management Support Coordinator
Location: Remote
Anticipated Start Date: 1/16/2025

Expected Duration: 3+ Months of contract with possible extension

Pay Range: $22 - $23/hr.

Job Description: Care Management Support Specialist
Position Purpose:
The Care Management Support Specialist is responsible for assisting with administrative care management activities, including performing outreach, answering inbound calls, and scheduling services. This position serves as a primary point of contact for members, providers, and staff to resolve inquiries, provide support, and document member records in compliance with current state and regulatory guidelines.
Education/Experience:
- Requires a High School diploma or GED
- Requires 1 - 2 years of related experience
Key Responsibilities:
- Outreach & Support: Provides outreach to members via phone to discuss care plan next steps, community or health plan resources, and address questions or concerns related to scheduling. Supports ongoing education for both members and providers throughout the care/service process.
- Resource Coordination: Connects members to health plan and community resources to ensure they are receiving comprehensive and high-quality customer care/service.
- Health Plan Knowledge: Applies a working knowledge of assigned health plan activities and resources to assist with member inquiries, including providing details about benefits, procedures, and available resources.
- Member/Provider Inquiries: Serves as the front-line support for various inquiries, requests, or concerns from members and/or providers, including explaining care plan procedures, protocols, and services.
- Onboarding & Administrative Support: Assists with member onboarding and day-to-day administrative tasks, such as sending welcome letters, program-related correspondence, and educational materials to support a successful member/provider relationship.
- Documentation & Compliance: Maintains accurate non-clinical member records, ensuring compliance with state and regulatory requirements, and ensures timely and accurate sharing of relevant information with providers as needed.
- Social Determinants of Health (SDOH): Identifies and refers members to community resources to address Social Determinants of Health (SDOH) needs, ensuring holistic care coordination.
- Other Duties: Performs other administrative duties as assigned, contributing to the efficient functioning of the care management team.
- Policy Compliance: Adheres to all company policies, standards, and regulatory requirements to ensure the highest level of service and compliance.
Skills & Qualifications:
- Excellent communication and interpersonal skills, with the ability to engage with members, providers, and team members professionally and effectively.
- Knowledge of health plan resources and community services, with the ability to make appropriate referrals.
- Strong organizational skills and attention to detail.
- Ability to handle multiple tasks efficiently and maintain accurate documentation.
- Familiarity with regulatory requirements related to healthcare services and documentation.
- A customer-centric attitude, with a focus on delivering high-quality service to members.
