BCforward is looking for Care Management Support Coordinator II Remote
Job Title: Care Management Support Coordinator II
Location: Remote (Pacific Time Zone)
Assignment Duration: 6 Months (Possible Extension or Conversion)
Pay Rate: $23/hr
Target Start Date: May 26, 2025
Work Schedule: Monday - Friday, 8:00 AM - 5:00 PM PST (No Overtime)
Position Purpose:
The Care Management Support Coordinator II provides vital administrative support to care management teams by conducting outreach, handling inbound calls, scheduling services, and coordinating member services. This role acts as the front-line liaison for members, providers, and internal staff, helping ensure high-quality service and compliance with state and regulatory standards.
Key Responsibilities:
- Perform outreach to members regarding care plans, community resources, appointment scheduling, and health education.
- Support member onboarding processes by sending welcome letters, educational materials, and other correspondence.
- Answer inbound calls and resolve issues or direct inquiries related to care coordination.
- Connect members with appropriate internal departments or community services to meet healthcare and Social Determinants of Health (SDOH) needs.
- Maintain and update accurate non-clinical member records in accordance with regulatory and internal guidelines.
- Apply knowledge of health plan benefits and services to support member needs.
- Submit referrals and coordinate care communication, including preparing letters or documents as required.
- Comply with company policies, procedures, and performance standards.
- Perform other duties as assigned.
Required Qualifications:
- High School Diploma or GED
- 1-2 years of experience in a healthcare customer service, administrative, or support role
- Strong verbal and written communication skills across all organizational levels
- Ability to work independently or collaboratively in a fast-paced environment
- Proficiency with Microsoft Office Suite (Word, Outlook, Excel)
Preferred Qualifications:
- Associate's degree (AA/AS)
- Bilingual or multilingual proficiency
- 2-5 years of customer service experience in healthcare settings
