Care Coordinator II
BCforward is currently seeking a highly motivated Care Coordinator II for an opportunity Remote!
Position Title: Care Coordinator II
Location: Remote, FL (Must reside in Florida)
Expected Duration: 06 Months of contract with possible extension
Job Type: [FULL TIME (>%3D40 HRS WEEKLY], [CONTRACT], [Remote]
Schedule: 5 PM - 8 AM EST
Targeted Start date: 5/12/2025
Pay Rate: $18-$22 per hour
Position Purpose:
The Care Coordinator II plays a crucial role in supporting care management activities for members, ensuring that healthcare providers and partners deliver services effectively, and achieving continuity of care/member satisfaction. The role involves interacting with members through telephonic outreach and home visits, and documenting care plans to ensure the delivery of appropriate services and resources.
Key Responsibilities:
- Member Outreach: Perform outreach to members via phone or home visits to discuss care plans/service plans, answer questions, address concerns, and provide education on available resources throughout the care process.
- Care Coordination: Work closely with healthcare providers, community partners, members, and caregivers to coordinate care activities based on the care plan/service plan. Make adjustments based on any progress or changes in the member's condition.
- Support for Inquiries: Provide support for any inquiries, requests, or concerns from members or providers related to care plans or service plans.
- Collaboration: Communicate with care managers, practitioners, and other team members to facilitate member services and ensure continuity of care.
- Assessment and Documentation: Assist with performing service assessments and screenings to evaluate the member's care needs, and document all necessary information to maintain accurate and up-to-date records in compliance with state and regulatory guidelines.
- Referrals and Education: Identify needs and make referrals to Care Managers, Disease Managers, or community organizations. Educate members on benefits, resources, and the care process.
- Compliance: Adhere to all policies, standards, and regulatory requirements, ensuring that all activities are compliant with contractual and regulatory guidelines.
- Additional Duties: Perform other related duties as assigned by management.
Qualifications:
Education/Experience:
- High School diploma or GED required.
- 1-2 years of relevant experience in a healthcare, customer service, or care coordination setting.
Preferred Qualifications:
- Experience in a managed care setting, medical office, or healthcare facility with medical administration duties.
- Familiarity with medical terminology and utilization review or claims processing practices.
- Bilingual candidates are a plus.
Skills:
- Strong communication and problem-solving abilities.
- Proficiency in Microsoft Office (basic understanding required).
- Ability to work empathetically with families and children, especially those dealing with complex medical conditions.
- Knowledge of customer service principles and healthcare delivery processes.
