BCforward is currently seeking a highly motivated Care Management Support Coordinator II for an opportunity Remote (Preferred candidates from WA, OR, NV, or AZ in MST or PST time zones)!

Job Title: Care Management Support Coordinator II

Location: Remote (Preferred candidates from WA, OR, NV, or AZ - MST/PST time zones)

Pay Rate: $25/hr

Target Start Date: 03/05/2025

Job Type: Contract (Potential to Extend)

Work Schedule:

  • Monday - Friday, 8:00 AM - 5:00 PM PST (No flexibility in schedule).
  • Overtime may be required as needed.
Position Purpose:

The Care Management Support Coordinator II plays a crucial role in supporting administrative care management activities, including outreach, inbound call handling, and service scheduling. The position serves as a primary contact for members, providers, and internal staff to resolve issues while ensuring compliance with state and regulatory guidelines.

Key Responsibilities:

  • Conduct outbound calls to existing members to discuss their care plans, enroll them in the Care Management program, and provide necessary information.
  • Support members by connecting them to health plan and community resources to enhance the quality of customer care and services.
  • Serve as the first point of contact for member/provider inquiries, including care plan procedures and protocols.
  • Assist with onboarding new members by sending welcome letters, educational materials, and program correspondence.
  • Maintain and update non-clinical member records following state and regulatory requirements.
  • Understand existing benefits and available local resources, making referrals to address Social Determinants of Health (SDOH) needs.
  • Perform data entry into TrueCare and navigate multiple systems efficiently.
  • Work in a production-based environment, making at least 60 calls per day in a fast-paced setting.
  • Adapt to changes in role responsibilities based on business needs.

Required Qualifications:

  • High School diploma or GED.
  • Minimum of 1-2 years of experience in a high-volume, fast-paced call center environment.
  • At least 2 years of healthcare experience.
  • Strong data entry skills and ability to navigate multiple systems.
  • Excellent communication and customer service skills.
  • Ability to meet performance metrics, including call volume and program enrollments.

Preferred Qualifications:

  • Sales experience (ability to deliver persuasive pitches to members).
  • Experience with outbound sales or customer engagement.
  • Proficiency in Microsoft Excel for basic reporting.
  • Bilingual in Spanish is a plus.
  • Flexibility to adapt to changing call types and business needs.

Work Environment:

  • Fully remote role (No on-site work at Centene facilities).
  • Candidates must reside in MST or PST time zones (WA, OR, NV, AZ preferred).

Performance Expectations:

  • Maintain a high call volume (minimum of 60 outbound calls per day).
  • Achieve enrollment targets for the Care Management program.
  • Ensure high-quality customer service and compliance with regulations.
  • Adapt to business needs and changes in call types.

Ideal Backgrounds:

  • Call Center Representative, Customer Service Representative (CSR), Customer Support Associate (CSA), Outbound Sales Representative.
