BC Forward is looking for Care Management Support Coordinator II (Remote)

Position Title: Care Management Support Coordinator II (Remote)

Locations: Remote - NY (EST)

Anticipated Start Date: 12/30

Expected Duration: 2 months

Job Type: Contract with extension and possible Temp to Perm.

Pay Rate: $19.00/hr on W2

Shift: 8:30AM-5PM EST. Monday-Friday

Need: Min 2 yrs of Previous Healthcare Experience.

Job Description:

Position Purpose:
Supports administrative care management activities, including performing outreach, answering inbound calls, and scheduling services. Acts as a point of contact for members, providers, and staff to resolve issues and document member records in compliance with state and regulatory guidelines.


Required Skills/Experience:

  • Strong customer service skills
  • Experience with metrics and performance tracking

  • Preferred Skills/Experience:

  • Previous healthcare experience

  • Education/Experience:
    • High School diploma or GED required.
    • 1-2 years of related experience preferred.

    Walk me through the day to day responsibilities of this the role and a description of the project (Outside of Workday JD):
    • This individual would be making calls to our members to verify their home care services.
    Describe the performance expectations/metrics for this individual and their team:
    • We would expect this individual to make 25 calls minimum per day to our members.
    Tell me about what their first day looks like:
    • There would be a training for all of our systems, and we would then have job specific training in a virtual classroom
    Key Responsibilities:
    • Provides outreach to members via phone to assist with care plan next steps, community or health plan resources, and to address scheduling questions or concerns.
    • Offers ongoing education and support to both members and providers throughout care/service.
    • Connects members with health plan and community resources to ensure high-quality customer care and service.
    • Applies working knowledge of assigned health plan(s) activities and resources.
    • Serves as the first line of support for member and provider inquiries, including explaining care plan procedures and protocols.
    • Supports member onboarding and day-to-day administrative duties, including sending out welcome letters, correspondence, and educational materials to foster a successful member/provider relationship.
    • Documents and maintains non-clinical member records, ensuring compliance with state and regulatory requirements, and provides records to providers as necessary.
    • Knowledge of local benefits and resources to address Social Determinants of Health (SDOH) needs, making appropriate referrals.
    • Performs other duties as assigned.
    • Complies with all policies, standards, and guidelines.
