- High School diploma or GED required.
- 1-2 years of related experience preferred.
Walk me through the day to day responsibilities of this the role and a description of the project (Outside of Workday JD):
- This individual would be making calls to our members to verify their home care services.
- We would expect this individual to make 25 calls minimum per day to our members.
- There would be a training for all of our systems, and we would then have job specific training in a virtual classroom
- Provides outreach to members via phone to assist with care plan next steps, community or health plan resources, and to address scheduling questions or concerns.
- Offers ongoing education and support to both members and providers throughout care/service.
- Connects members with health plan and community resources to ensure high-quality customer care and service.
- Applies working knowledge of assigned health plan(s) activities and resources.
- Serves as the first line of support for member and provider inquiries, including explaining care plan procedures and protocols.
- Supports member onboarding and day-to-day administrative duties, including sending out welcome letters, correspondence, and educational materials to foster a successful member/provider relationship.
- Documents and maintains non-clinical member records, ensuring compliance with state and regulatory requirements, and provides records to providers as necessary.
- Knowledge of local benefits and resources to address Social Determinants of Health (SDOH) needs, making appropriate referrals.
- Performs other duties as assigned.
- Complies with all policies, standards, and guidelines.
