BCForward is looking for Care Management Support Coordinator II- Remote
Job Title: Care Management Support Coordinator II
Work Location: Remote
Duration: 03+ Months
Potential to Extend: Yes
Shift: 8:00 AM - 5:00 PM Monday - Friday PST (No flexibility in schedule)
Pay Rate: $25.00/hr on W2
Position Purpose:
The Care Management Support Coordinator II will support the Care Management team for the CA Market by making outbound calls to existing members, discussing their care needs, and enrolling them into the Care Management program. This is a fast-paced, production-based role within a call center environment, requiring high call volume and the ability to adapt to varying tasks.
Responsibilities:
- Make outbound calls to existing members, discussing care needs and encouraging enrollment into the Care Management program.
- Achieve production targets based on the number of members enrolled.
- Make a minimum of 60 calls per day in a fast-paced environment.
- Enter data into True Care and navigate multiple systems.
- Report basic data using Excel.
- Adapt to changing call types based on business needs while remaining flexible.
- Provide excellent customer service and sales pitches to existing members.
- Ensure the accuracy of all information entered into the system.
