BC Forward is looking for Care Management Support Coordinator II (Remote) in Kentucky

Position Title: Care Management Support Coordinator II (Remote)

Locations: REMOTE - KY (EST OR CST)
Anticipated Start Date: 02/24
Expected Duration: 6 months
Job Type: Contract with possible extension
Work Schedule: Typical shifts include: 8:00 AM - 5:00 PM / 8:30 AM - 5:30 PM / 9:00 AM - 6:00 PM Start times are flexible, with openness to later start times beyond 9:00 AM if needed.
Pay Rate: $22.00/hr. on W2.
Need: Prior experience in a medical office or healthcare setting and High Volume Calling with Medical training, certification, or education in mental health.



Job Description:
The role supports administrative care management activities by conducting outreach, managing inbound calls, and coordinating services. It serves as a central point of contact for members, providers, and staff, addressing inquiries and resolving issues. The position ensures accurate documentation of member records in compliance with state and regulatory guidelines.

Day-to-Day Responsibilities:
  • Data Management and Member Outreach:
    • Extract member information from a computer system and compile it into a spreadsheet for tracking and reporting.
    • Conduct outreach calls to members to address care gaps in specific areas, assisting them with tasks such as scheduling primary care provider (PCP) appointments and finding a PCP when necessary.
    • Identify and connect members with community resources to address Social Determinants of Health (SDOH) needs.
    • Accurately document all interactions, progress, and follow-ups in multiple systems to ensure comprehensive records of member support activities.

Performance Expectations and Metrics:
  • Call Volume and Member Engagement:
    • Achieve a weekly call target of 250 calls, with a daily focus on managing a panel of 50-60 members.
    • Effectively engage with members to close care gaps, ensuring timely and effective assistance with care coordination and resource navigation.
  • Documentation and Compliance:
    • Maintain detailed and accurate documentation of all activities and interactions in compliance with organizational standards and regulatory requirements.
  • Audit Scores:
    • Consistently meet or exceed monthly audit score expectations, demonstrating accuracy, efficiency, and adherence to established processes.
Required Skills and Experience:

  • Prior experience in a medical office or healthcare setting.
  • Familiarity with medical terminology.
  • Basic computer proficiency, including the ability to use software for documentation and outreach.

  • Preferred Skills and Experience:

  • Experience working with community-based organizations.
  • Knowledge of medical screenings and their purpose.
  • Additional medical training, certification, or experience in mental health is highly desirable.

  • Education Requirements:
    • Required: High School Diploma or GED.
    • Preferred: Medical training, certification, or education in mental health.
    Software Skills:
    • Proficiency in Microsoft Outlook and Excel is required.
    Key Responsibilities:
    • Member Outreach and Support:
      • Contact members via phone to assist with care plan follow-ups, address questions or concerns, and provide ongoing education for both members and providers.
      • Connect members with health plan and community resources to ensure they receive high-quality care and support.
    • Provider and Member Assistance:
      • Act as a frontline resource for member and provider inquiries, requests, or concerns, including explaining care plan procedures and protocols.
      • Facilitate member onboarding by sending welcome letters, educational materials, and correspondence to support strong member-provider relationships.
    • Administrative and Documentation Tasks:
      • Maintain and update non-clinical member records in compliance with state and regulatory requirements.
      • Provide accurate and timely documentation to providers as needed.
      • Assist with day-to-day administrative duties to streamline care management processes.
    • Resource Navigation:
      • Apply knowledge of available health plan activities and community resources to guide members.
      • Make appropriate referrals to address Social Determinants of Health (SDOH) needs and connect members to relevant services.
    • Compliance and Standards:
      • Adhere to all policies, procedures, and regulatory standards to maintain service quality and accountability.
    • Other Duties:
      • Perform additional tasks and responsibilities as assigned to support care management activities.

    Education and Experience:
    • Required:
      • High School diploma or GED equivalent.
      • 1-2 years of related experience in administrative support, care management, or customer service.
    • Preferred Skills and Qualifications:
      • Familiarity with care plans, health plan resources, and community services.
      • Strong communication and problem-solving skills.
      • Ability to manage multiple tasks efficiently and provide excellent customer service.
