BCforward is currently seeking a highly motivated Care Coordinator II for an opportunity in Remote!

Position Title: Care Coordinator II

Location: Remote (CST)

Anticipated Start Date: 01/13/2024

Expected Duration: 6 Months

Job Description:

Position Purpose:

Support care management activities and ensure seamless delivery of services by healthcare providers and partners to enhance continuity of care and member satisfaction. This role involves member outreach, coordination of care activities, and documentation of service plans in compliance with regulatory standards.

Key Responsibilities:

  • Conduct member outreach via phone or home visits to discuss care/service plans, address concerns, and provide ongoing education.
  • Coordinate care activities with healthcare providers, community partners, and members/caregivers to adapt to changes or progress in the care plan.
  • Assist with member and provider inquiries related to care plans or services, ensuring prompt and accurate resolution.
  • Communicate with care managers, practitioners, and others to facilitate member services and ensure continuity of care.
  • Perform service assessments and screenings as needed, documenting care needs in compliance with regulatory requirements.
  • Maintain accurate member records, distributing them to providers when necessary.
  • Adhere to standards of practice, contractual obligations, and regulatory guidelines.
  • Identify member needs and make referrals to care managers, community organizations, and disease management resources.
  • Provide education on available benefits and resources.
  • Perform other duties as assigned.
Qualifications:

Education: High School diploma or GED required; Bachelor's degree preferred but not mandatory.

Experience:

  • 1-2 years of related experience.
  • 2-3 years of managed care or physician's office experience preferred.
  • Customer service experience, especially in phone-based roles.
  • General proficiency in MS Office (Excel, Word, Outlook).
  • Experience working directly with healthcare providers and patients preferred.
  • Scheduling experience is a plus.
Daily Responsibilities:

  • Engage with disenrolled members to encourage re-enrollment with the health plan.
  • Assist with medical and psychosocial aspects of utilization and coordinated care.
