Job Description
We are seeking a Care Coordinator II to join our team. The candidate will have experience in care coordination, member outreach, and documentation and a proven ability to support continuity of care and member satisfaction.
Responsibilities:
- Conduct outreach to members via phone or home visits to review care and service plans, next steps, resources, and questions or concerns.
- Coordinate care activities per the care or service plan with healthcare and community providers, partners, members, and caregivers.
- Support member and provider inquiries related to care or service plans and escalate as appropriate.
- Communicate with care managers, practitioners, and interdisciplinary team members to facilitate services and ensure continuity of care.
- Perform service assessments or screenings for members and document care needs, as applicable.
- Document and maintain accurate member records per state and regulatory requirements and distribute to providers as needed.
- Follow standards of practice, policies, and contractual and regulatory guidelines.
- Identify member needs and make referrals to Care Managers, Disease Managers, and community-based organizations.
- Provide education on available benefits and community resources.
- Perform other duties as assigned and comply with all policies and standards.
Required Skills & Qualifications:
- High School diploma or GED.
- 1 to 2 years of related experience in care coordination, case management support, or healthcare customer service.
- Proficiency in member outreach by phone and in person with clear documentation skills.
- Ability to collaborate with providers, community resources, and internal care teams.
Preferred Skills:
- Familiarity with Medicaid, Medicare, or managed care environments.
- Experience with electronic health records or care management platforms.
