Responsibilities
- Provides outreach to members via phone or home visits to engage members, discuss care/service plans, and review next steps, resources, questions, or concerns related to recommended care. Provides ongoing education as appropriate throughout care/service.
- Coordinates care activities based on the care/service plan, collaborating with healthcare providers, community partners, members, and caregivers to adjust for changes or progress as needed.
- Serves as support for various member and/or provider inquiries, requests, or concerns related to the care/service plan.
- Communicates with care managers, practitioners, and others to facilitate member services and ensure continuity of care.
- May support performing service assessments/screenings for members and documenting care needs.
- Maintains member records in accordance with state and regulatory requirements and distributes to providers as needed.
- Follows standards of practice and policies compliant with contractual requirements and regulatory guidelines.
- Identifies member needs and makes referrals to Care Managers, community-based organizations, or Disease Managers.
- Provides education on benefits and resources available.
- Performs other duties as assigned.

- New hires meet with a supervisor to review expectations and department overview. After orientation, they are paired with a preceptor for 1:1 training (beginning Day 1).
- During the first month, new hires meet weekly with leadership to discuss training and performance.
- The team prides itself on open communication, daily chats, and team support during onboarding and beyond.
Typical Day in the Role
- Conduct outbound quarterly calls to members for follow-up questions.
- Assigned cases per month.
- Complete research (e.g., identifying member information, connecting with providers).
- Make 3 contact attempts per member (Day 1, Day 2, Day 3). If unsuccessful, connect with PCP to confirm correct contact details.
- May be assigned stretch projects based on volume.
- Expected to complete 50-75 calls per day.
- Submit daily productivity reports via Outlook.
- Remote work environment: if unable to work from home due to power, equipment, or internet issues, employee will be required to report to the office.

- Previous work-from-home experience

- Missing relevant experience (e.g., Centene contract experience, recent work experience)

- Ability to multitask (calls + note-taking simultaneously)
