Care Management Support Coordinator II
Position: Care Management Support Coordinator IILocation: Remote - Must reside in DE, PA, NJ, or MD (occasional onsite meetings)Duration: 4 months (possibility to extend or convert)Pay Rate: $22.00/hr on W2Schedule: Monday - Friday, 8:00 AM - 5:00 PM (No OT without approval) Position PurposeThe Care Management Support Coordinator II provides administrative support for care management activities, serving as a primary point of contact for members, providers, and staff. This role focuses on outreach, scheduling, and connecting members to community and health plan resources to ensure high-quality care and compliance with state and regulatory requirements. Key Responsibilities
- Conduct outreach calls to members, providers, and pharmacies to assist with care plan follow-up, scheduling, and care coordination.
- Support members in accessing community and health plan resources, addressing social determinants of health (SDOH) needs.
- Serve as front-line support for member and provider inquiries, explaining care plan procedures and resources.
- Document and maintain non-clinical member records, ensuring compliance with policies and regulatory standards.
- Assist members with appointment linkage, finding primary care providers (PCPs), and enrolling in care coordination services.
- Send welcome letters, correspondence, and educational materials to facilitate positive member and provider relationships.
- Perform general administrative duties as assigned.
- High School diploma or GED.
- 2-3 years of experience in a managed care setting, medical office, or similar healthcare environment.
- Experience handling high call volumes (50+ calls daily, inbound/outbound).
- Strong communication, organizational, and computer skills (including EMR systems).
- Familiarity with medical terminology.
- Knowledge of community health resources and benefits.
- Understanding of social determinants of health (SDOH).
- No prior experience in healthcare, medical settings, or managed care environments.
- Meeting outreach and documentation performance metrics.
- Demonstrated ability to resolve member and provider inquiries efficiently.
- Strong teamwork and collaboration in a remote environment.
