Pay Rate: $25/hour
Schedule: Monday - Friday, 9:00 a.m. - 6:00 p.m. EST
Position PurposeThe Care Coordinator II supports care management activities and assigned member teams to ensure services are delivered by healthcare providers and partners in alignment with care plans. This role ensures continuity of care, service quality, and member satisfaction by conducting outreach, coordinating services, and documenting member interactions telephonically or through home visits. Position PurposeThe Care Coordinator II supports care management activities and assigned member teams to ensure services are delivered by healthcare providers and partners in alignment with care plans. This role ensures continuity of care, service quality, and member satisfaction by conducting outreach, coordinating services, and documenting member interactions telephonically or through home visits. Key Responsibilities
- Conduct outbound and inbound calls with members to discuss care/service plans, next steps, available resources, and address questions or concerns.
- Coordinate care activities among healthcare providers, community partners, and members/caregivers based on the service plan.
- Support member and provider inquiries or requests related to the care plan/service plan.
- Communicate with care managers and practitioners to ensure continuity of care and timely services.
- Perform service assessments and screenings as needed and document care needs.
- Maintain and update member records per state, regulatory, and contractual requirements.
- Identify member needs and refer to appropriate Care Managers, Disease Managers, or community-based organizations.
- Educate members on available benefits and healthcare resources.
- Submit daily productivity reports via Outlook.
- Comply with company policies, standards, and privacy regulations.
- Perform additional duties as assigned.
- Required: High School Diploma or GED
- Preferred: Associate degree or higher in a healthcare-related field
- Experience: 1-2 years of related experience; 2+ years in healthcare or managed care strongly preferred
- Florida (Sunshine Health Plan): All member interactions are telephonic.
- Driver's License: Not required for this role (Florida-based remote position).
- 2+ years of experience in healthcare or managed care
- Comfortable with high-volume outbound/inbound calls (50-75 calls/day)
- Proficient with general technology tools (Excel, Outlook, Word, Teams)
- Dedicated, private workspace for remote work
- Bilingual (Spanish or Creole preferred)
- Prior work-from-home experience
- Unexplained employment gaps (6-8 months) without context on resume
- Missing educational information (all degrees must be listed)
- Lack of relevant healthcare or call volume experience
- Accuracy and timeliness in documenting calls and care coordination
- Ability to multitask effectively (calling while taking notes)
- Professional communication and appearance in remote setting
- Adherence to confidentiality and compliance standards
- Fully remote role (must reside in Florida)
- Occasional in-person meetings (quarterly) preferred in the Tampa Bay area
- If unable to work remotely due to power/internet issues, must report to the local office
- Team culture emphasizes open communication, collaboration, and support
- 1:1 preceptor-led training during the first two weeks
- Weekly check-ins with supervisor during the first month
- Ongoing mentorship and access to team resources
- Training follows preceptor schedule (typically 8-5 or 9-6)
