BCForward is currently looking for Care Coordinator II in Tampa, FL (Remote)
Job Title: Care Coordinator II
Location: Tampa, Florida (Remote)
Expected Duration: 6 months, Contract-to-Hire
Target Start date: ASAP
Schedule: Monday - Friday, 9:00 a.m. - 6:00 p.m. EST
Pay rate: $25.00/hr on W2
Position Purpose:
The Care Coordinator II supports care management teams by facilitating continuity of care for health plan members. This includes performing telephonic outreach and, when needed, home visits to engage members, educate them on care plans, and ensure delivery of healthcare services. The goal is to enhance member satisfaction, ensure coordination between providers, and support members throughout their care journey.
Key Responsibilities:
- Conduct telephonic or in-person outreach to engage members and review their care/service plans.
- Explain care plan next steps, answer questions, and provide education on available services and resources.
- Coordinate care activities with healthcare providers, community partners, and caregivers.
- Assist with member or provider questions related to care/service plans.
- Communicate with care managers and clinical teams to ensure continuity of services.
- Support service assessments and help document member needs and care requirements.
- Maintain accurate and up-to-date member records per state and regulatory requirements.
- Identify member needs and refer to Care Managers, community organizations, or Disease Management as appropriate.
- Educate members on their health plan benefits and available support resources.
- Submit daily call and activity reports via Outlook.
- Adhere to all organizational policies and quality standards.
- Perform other duties as assigned.
Education & Experience Requirements:
- Required: High School diploma or GED
- Experience: Minimum 1-2 years in a healthcare or managed care setting
- Preferred: Previous remote work experience
Ideal Candidate Profile:
Must-Haves:
- 2+ years in a healthcare or managed care setting
- Comfortable with high-volume inbound and outbound calling (50-75 calls/day)
- Solid technical skills - Microsoft Excel, Outlook, Word, Microsoft Teams
- Strong communication, problem-solving, and organizational skills
- Ability to work independently and maintain professionalism in a remote environment
- Dedicated home office setup, free from distractions
Nice-to-Haves:
- Bilingual (Spanish or Creole)
- Prior experience working remotely
- Knowledge of HIPAA and healthcare confidentiality best practices
