BCforward is currently seeking a highly motivated Care Manager (RN) for an opportunity in Remote-IN!

Pay Range: [$45.00] - [$48.09]

  • 2-4 years of nursing experience
  • Required: OB-related experience (Prenatal, Postpartum, Labor/Delivery, Mother Baby, or NICU)

Job Description:

Position Overview:

Join our OB Care Management team to support high-risk pregnant Medicaid members in Indiana. As a Care Manager (RN), you will develop and implement person-centered care plans, provide education, and coordinate services that address medical and social needs to improve outcomes and support health equity.

Key Responsibilities

  • Evaluate and assess member needs, including physical, behavioral, and social health factors
  • Develop and monitor personalized care/service plans in collaboration with providers
  • Perform telephonic outreach to members (15-30 calls/day)
  • Address barriers to care and provide community resource referrals (housing, transportation, baby supplies, etc.)
  • Educate members on pregnancy, health conditions, and health plan benefits
  • Ensure timely documentation that meets NCQA and audit standards (monthly chart audits must meet or exceed 94%)
  • Participate in team huddles and care rounds with OB team and leadership
  • Engage in 3-week onboarding/training and structured precepting period

Requirements

Education:

  • Degree from an accredited School of Nursing OR Bachelor's degree in Nursing
Experience:

  • 2-4 years of nursing experience
  • Required: OB-related experience (Prenatal, Postpartum, Labor/Delivery, Mother Baby, or NICU)
Licensure:

  • Active Indiana RN License or Compact State RN License
Skills & Tools:

  • Strong care coordination and communication skills
  • Experience with Microsoft Teams, Outlook, and Word
  • Detail-oriented and self-motivated; ability to work independently
  • Must be comfortable in a remote environment with task-based organization
